We are a forward thinking and innovative Hospice at Homeservice and we are recruiting a forward-thinking innovative Specialist DementiaNurse to develop our Hospice services for patients and their carers. The post is for 30 hours workingacross North Northumberland you will be responsible for working collaborativelywith community services, Acute Admiral Nurses, NHS and other providers, inensuring that patients and carers are able to access appropriate servicesfollowing a Dementia Diagnosis.

You will develop service provision within the Hospice,acting as the advocate for patients and carers, signposting to appropriateservices as well as providing direct care to patients. You will work inpartnership with the Specialist Dementia service in Tynedale Hospice at Home, sharingbest practice and influencing the Palliative and End of Life Dementia Pathwayacross Northumberland.

You will be working alongside our Hospice Registered Nurses,Hospice Support Workers, Pre and Post Bereavement Family Services to offer wraparound care to Dementia patients and their carers.

Main duties of the job

Job responsibilities

Main Responsibilities:

Manage own caseload of patients with Dementia, working closely with Care Coordinators to support consistency of care giver Undertake the function as Dementia lead for the Hospice including but not limited to evaluation of current care provision for patients with Dementia/utilisation of best practice for patients with Dementia/ development of the Hospice Dementia strategy Contribute to the prevention of inappropriate admissions into Acute care settings ensuring care delivery at home meets the needs of Dementia patient and their main carer Deliver education and training for staff in delivering evidence based best practice across Hospice care services Develop a team of Volunteer Dementia friends from our Volunteer community to provide a befriending service to patients with Dementia and their main carer Work collaboratively with NHS and other providers of Dementia services to ensureappropriate signposting for community services for patients and their main carer Evaluation and auditing of Hospice delivered services for patients with Dementia and their main carer. Contribute to collection and reporting of appropriate Key Performance Indicators for PEOLC. Person Specification

Experience of nursing/working with patients with Dementia Acute and Community nursing experience Familiarity with service reporting and Key Performance Indicators Experience undertaking audits Commitment to Dementia Care excellence Highly motivated self -starter Flexible in attitude to work, and undertaking of role Able to travel independently within West Northumberland

Desirable

Post qualification experience as a Specialist nurse within the community setting Post registration recognised Dementia academic study Experience in delivery of Hospice Care
