Job summary

We are currently recruiting for an Assistant Practitioner to join our community Paignton Health and Well-Being Team.

To work as a Assistant Practitioner within an integrated team, will result in your working across the following services: Community Nursing, Intermediate Care and Community Matrons.

Main duties of the job

You will deliver high quality care to the housebound patients of Paignton, and work as part of multi-disciplinary team in partnership with primary care and secondary care.

Foundation Degree (FdSc in Healthcare Practice (Nursing Associate) Trust induction training

Understand and demonstrate evidence-based practice Understanding of when to seek advice and refer to a registered care professional Understanding of the scope of the role of the Nursing Associate in context of the team and the organisation, and how the role may contribute to service development Intermediate IT skills Understand and demonstrate the importance of the promotion of health and wellbeing
