Patient Safety Improvement Manager

Job summary
To support the Head of Quality & Safety for Investigations and Quality Improvement in the provision and planning of safe high quality health care to patients across all areas of Cwm Taf Morgannwg University Health Board (CTMUHB), inclusive of the independent contractor services. In particular the operational management of patient safety incidents, contributing to concerns and complaints where required when linked to a patient safety incident, as well as Learning from Events Records and associated cases of legal claims .
The post holder will support the Head of Quality & Safety to plan and deliver robust systems and processes for ensuring that Concerns involving patient safety are appropriately reported, and investigated. The post holder will be proactive and dynamic in ensuring that lessons from Concerns and incidents are learned and shared widely, and that they contribute to continuous service improvement and the highest standards of patient care.
Main duties of the job
? Be responsible for supporting patient safety within specific Care Group. This will involve significant time working within Clinical Service Groups to provide support to the Heads of Nursing, within the scope of the role.? Support the development and implementation of appropriate corporate policies and procedures.? Work collaboratively with colleagues across all departments and clinical areas to provide expert advice and support on all aspects of patient safety.? Provide effective and responsive support to Care Group services, by co-ordinating and advising on the investigation of incidents and supporting appropriate investigations (including Root Cause Analysis) which have resulted in harm, advising on Duty of Candour, or where trends indicate the need for in-depth investigation, as agreed with the Head of Quality & Safety/Nurse Director /or Heads of Nursing.? Facilitate a Being Open approach in order to deliver the best outcomes for patients in line with Duty of candour requirements.

The post holder must appropriately organise and prioritise their own work, working collaboratively with Clinical Service Group colleagues to ensure that patient safety incidents are appropriately reported and managed across the Health Board, ensuring compliance with all relevant policies, procedures, processes and authorities such at PTR and Duty of post holder will ensure a sensitive and responsive approach in accordance with national guidelines and legislation, professional and regulatory requirements, and the Health Boards objectives.Support the Head of Quality & Safety via delegated responsibility in the development of policies and procedures, to implement Putting Things Right. In so doing they will ensure an approach which will maximise quality and patient safety and minimise clinical risk across all Care Groups and departments including the Independent Contractor Services, through the application of specialist knowledge and expertise.

Qualifications and/or Knowledge
Essential
Educated to Master's Degree level or relevant equivalent experience within a similar environment. Training in Root Cause Analysis. Understanding of Incident Management Process

Knowledge and experience of the administration and use of the Datix system.
