Job summary

As a Colorectal Cancer Clinical nurse specialist, you will perform a key role and be a core member in management and leadership of the Colorectal Multidisciplinary Team. You will be an advocate (Key worker) for patients that are diagnosed with colorectal cancer and continually look for new and innovative ways of improving the patient experience / pathway.

Be responsible for patient assessment, planning of care, giving, and supervising safe and compassionate evidenced based care, and evaluating its effectiveness.

Undertake clinical examination, order diagnostic investigations and initiate changes in management of treatment, when necessary, and apply autonomous decision making.

Support patients from diagnosis, through their treatment and play a key part in their follow-up. Linking in with other services in the hospital and supporting patient in endoscopy, oncology and as inpatients.

Main duties of the job

Be a core member of the Colorectal cancer MDT.

Linking into the Anal MDT, Complex Cancer (MDT) Hepatobiliary MDT and Neuroendocrine (NET) based at Southampton General Hospital.

Act as a key point of contact between primary, secondary, and tertiary care contributing to increased Trust clinical activity by developing new ways of working which reflect the requirements of the Cancer Survivorship Initiative. Prevent and / or reduce emergency presentations and unnecessary hospital admissions, and reduce hospital inpatient stays, whilst enabling access without delay back into the appropriate cancer pathway.

The post holder will be expected to participate in clinical audit, collate data on patient interactions/attendances, and maintain the appropriate databases.

Person Specification

Registered Nurse

Essential

Registered adult nurse with significant, relevant experience at band 5. Relevant teaching qualification Diploma/Degree in relevant area or evidence of study to that level. Demonstrates evidence of ongoing professional development. Evidence of relevant clinical experience at Band 5 preferably in an Oncology, Endoscopy or Colorectal/GI surgery. Demonstrate a good understanding of managing GI cancer patients from low to high risk with complex needs and their cancer pathway. Experience of multi-professional working. Demonstrates ability to use audit to improve quality. Involvement in developing guidelines/policies/pathways. Ability to work as an independent practitioner making autonomous decisions. Excellent communication skills (verbal, non-verbal and written) with sensitive information.

Desirable

Post registration training preferably in cancer/palliative care. Advanced Communication skills training. Psychological Level 2 qualification. Demonstrate a good understanding of managing GI cancer patients from low to high risk with complex needs and their cancer pathway. Physical assessment and history taking, assess GI cancer patients, plan care, deliver care and evaluate the effectiveness of care given. Experience of working within a Band 6 role.
