Elmwood Medical Centre is situated in an idyllic rural area of Buxton, Derbyshire on the doorstep of the famous Pavilion gardens. We are located 30-45 minute away from Stockport, Tameside, Manchester and Sheffield areas and has free on site car parking.

Applicants with experience of working in a primary care setting is advantageous.

Main duties of the job

The post holder will work as part of the nursing workforce supporting registered nurses and other clinicians in delivering high quality, person-centred and compassionate care to patients.

They will be part of the multi-disciplinary team delivering planned care to a group of patients working within the standards of proficiency outlined by the Nursing and Midwifery Council and in accordance with organisation's policies and protocols.

They will demonstrate the ability to work effectively as part of a team and provide support/guidance to less experienced staff/learners within the clinical environment as needed.

They are required to act within their professional boundaries, supporting and working alongside a multi-disciplinary team within general practice and the Primary Care Network (PCN).

The post holder will contribute to providing nursing care to patients registered in the GP practices.

The post holder will focus on assuming a lead role in specified areas, in line with the Quality and Outcome Framework (QOF), to support quality health outcomes, including health promotion and the well-being of our patients.

The post holder will work with other members of the Multidisciplinary Team (MDT) to both organise and facilitate care appropriate to the individual needs of the patients.

Job responsibilities

Key duties and responsibilities include:

You will be providing care to people of all ages, in a busy and diverse practice, contributing to core nursing roles and responsibilities, supporting the registered nurses to allow them to focus on more complex clinical care.

Following appropriate training, the following competency-based skills maybe undertaken by qualified nursing associates working within general practice:

Adult and child Immunisation under a Patient Specific Direction. Live nasal flu vaccines under a Patient Specific Direction. Travel vaccines under a Patient Specific Direction. Shingles vaccine under a Patient Specific Direction. Pneumococcal vaccine under a Patient Specific Direction. B12 injections under a Patient Specific Direction. Cervical Screening undertaking the role of cervical sample taker (in line with Public Health guidance on training requirements). Taking of ECGs Phlebotomy including home visits if required (the correct indemnity and car insurance will be required). Monitoring of long-term conditions - QOF related reviews of complex patients. Participate in clinical and non-clinical audit. New patient health checks. Spirometry. Chaperoning duties as and when required. Home visits as and when required. Processing and management of laboratory samples requested by GPs/Nurses. Vaccine/cold chain storage, monitoring and recording. Surgical equipment and vaccine re-stocking and stock rotation. Clearing and re-stocking consulting rooms. Preparing and maintaining environments and equipment before, during and after patient care interventions including assisting GPs/PAs/Nurses as required. Assisting in the assessment and surveillance of patients health and wellbeing. Supporting and raising awareness of health and well-being and how it can be promoted. Assisting with the collection and collation of data on needs related to health and well-being. Collection of sample(s) of blood in accordance with policies, procedures and training. Liaising with and assisting members of the nursing team. Maintain an active status on the NMC register. Engage in public health, community development and in the reduction of health inequalities. Provide holistic, compassionate, safe and effective care and support to patients, their families and service users in a range of care settings under the direction of a registered nurse, following an agreed plan of care and without direct supervision. Safely administer medication in accordance with local and national guidance. Responsible for the correct and safe management of the specimens process including collection, labelling, handling, use of correct and clean containers, storage and transport arrangements. Chronic disease management including management of diabetic patients, COPD patients, asthmatics, patients with hypertension in line with the NICE guidance and practice protocols. To support the team in the assessment of patients with learning disabilities, mental health and dementia.

Administrative duties:

To have a thorough knowledge of service procedures and protocols, know where to access them for reference and adhere to them at all times. To support patient referrals ensuring robust follow up, whilst adhering to policies and protocols at all times. Support the practice team to undertake administrative tasks as required, following appropriate training if required. To attend meetings as and when required. Ensure adherence to the Data Protection Act at all times. To assist with the gathering of statistics and information when required.

Other duties:

Any other duties, as agreed with the line manager to meet the needs of the organisation. This may include travel to other sites within the organisation. Person Specification

Qualifications

Essential

Registered Nursing Associate on the NMC register. Nursing Associate Foundation Degree qualification. Demonstrable commitment to professional development.

Desirable

Qualified in Phlebotomy.

Experience

Essential

Understands and acts in line with NMC professional standards for practice contained within The Code. Understands the requirements of the NMC revalidation. Working in a healthcare setting. Knowledge of own strengths and weaknesses - escalating to the appropriate professional for expert help and advice. Dealing with the public/patients. Working as part of a team.

Desirable

Working with Clinical software EMIS Undertaking home visits.
