Job summary

An exciting opportunity has arisen for an enthusiastic and motivated Speech and Language Therapist to join our friendly and innovative multidisciplinary team (MDT).

You will have specialist experience in adult acquired dysphagia and a keen interest in rehabilitation and complex feeding issues. You will have expert assessment skills, including FEES competencies.

You will have excellent communication and organisational skills to manage a diverse caseload of adults as well as effective team working skills.

You will be an integral member of our established MDT; based and working closely with Dietitians, Nutrition Nurse Specialists, Speech and Language Therapists, Administrators and Support Workers.

Post is based in the Community at Waldron Health Centre, Amersham Vale

Main duties of the job

The Home Enteral Nutrition Team is a specialist service that provides expert support for people of all ages to safely manage their eating, drinking and tube feeding at home in Lambeth, Southwark and Lewisham.

The Nutrition and Dietetics Department and Speech and Language Therapy Department at Lewisham and Greenwich NHS Trust, provide a range of inpatient, outpatient and community services. You will also work closely with healthcare professionals within acute, community and tertiary settings.

Job responsibilities

Key Result Areas & Performance:

To demonstrate highly specialist clinical skills and knowledge in the assessment, treatment and management of swallowing problems in patients with feeding tubes living within Lambeth, Southwark and Lewisham, underpinned by current evidence-based practice. To make appropriate differential diagnoses and specialist clinical decisions on evidence from assessment of complex cases. To provide specialist advice and second opinions to non-specialists and other specialists/professionals within the field of dysphagia. To demonstrate specialist clinical skills for interpretative assessment techniques and interventions in the care of patients with tracheostomies To demonstrate specialist clinical skills for interpretative assessment techniques and interventions involving instrumental procedures including Videofluoroscopy and FEES To apply specialist knowledge to inform sound clinical judgements/decision making for case management and to develop clear care plans based on best practice To participate in MDT decision-making process around consent to treatment ( PEG and non-oral feeding) when capacity is questionable, being aware of the legal and ethical considerations. To adapt practice to meet individual patients circumstances, including due regard for cultural and linguistic differences. To demonstrate clinical effectiveness reflecting on evidence-based practice and outcome measures To employ counselling skills with patients, carers/ clients with highly complex needs including those related to terminal illness, death and severe disability or disfigurement. To manage a specialist clinical caseload of complex patients with the full responsibility for choosing from a wide range of evidence-based assessment and treatment techniques. To be fully responsible for own time and caseload management. To be accountable for own professional actions and recognise own professional boundaries, seeking and acting upon advice as appropriate.

Qualifications & Training

Essential

Degree or Diploma recognised by Royal College of Speech and Language Therapists and HCPC HCPC state registration. Member of RCSLT Postgraduate qualification in Dysphagia or extensive clinical experience FEES Level 2a Competent
