Job summary

This is an exciting opportunity to take a senior role in the West Essex Hospital at Home and Urgent Community Response Service.

The post holder will work with the senior management team to provide comprehensive, operational, staffing and budget management. They will be responsible for the efficient and effective day-to-day running of the Hospital at Home (HatH) and Urgent Community Response Service. This will include the delivery of operational performance targets (finance, activity, workforce and quality/safety) along with the achievement of service objectives and compliance with local and national standards relevant to the service area.

As a member of the leadership team, the post holder will share responsibility for the implementation of all Trust policies and the service improvement agenda within their area. As Matron, they will also provide clinical and professional guidance within areas of responsibility regarding operational issues. This will include being the Lead for Utilisation, Admission process, Discharge Planning and Length of Stay management. The post holder will also be expected to load on the investigation of complaints as directed by the senior management team.

Main duties of the job

To be responsible for all aspects of operational management of the HatH service To ensure all patients receive safe and compassionate care and experience a great service To lead a team of multi-disciplinary clinical staff, ensuring resources are used to maximise the benefit to all patients Take a lead in developing specialist skills within the team to help reduce the demand on acute sector care Ensure clinical care is appropriately monitored and audited and learning is shared across the organisation Lead daily MDTs and contribute to case conferences ensuring there is a holistic assessment of patients as required To set outcome based targets for the service and manage performance against these To respond to informal and formal complaints received in line with Trust policy; make changes to services, systems, policies, practice and communication as appropriate in line with policies To be responsible for communicating highly complex information to staff in relation to service developments. This may include the communication of developments that will not be received positively, for example changes to service provision To network with and establish effective partnership working with key personnel

Job responsibilities

To effectively manage the budget for areas of responsibility To take the lead responsibility for the prevention of infection in line with Trust and DH policies and guidelines, ensuring that infection control measures are implemented by all staff Ensure clinical care is appropriately monitored and audited and learning is shared across the organisation Lead daily MDTs and contribute to case conferences ensuring there is a holistic assessment of patients as required Identify clinical priority areas and improve standards of care Ensure that the service meets the requirements of patients, partners and Commissioners within a contestable business environment To build good working relationships with all commissioners To set outcome based targets for the service and manage performance against these Undertake clinical supervision and performance reviews for all relevant staff Work at all times within the Professional Code of Practice Improve the quality of patient care through effective clinical leadership Undertake clinical work, training and personal development to maintain and develop clinical and leadership skills Ensure all staff are aware of clinical policies and practices and that these are adhered to To initiate and influence policy development within the service and the Trust To encourage innovation and develop new approaches to patient care To liaise with voluntary and other agencies with respect to support mechanisms available to patients, patient information, patient literature etc. To act as a clinical supervisor, coach and mentor to other Colleagues

You will be expected to work collaboratively with key partner organisations, service users, carers, clinicians and other practitioners within the multi-disciplinary team in delivering services; providing a 7-day a week, 24-hours a day service for 365 days a year working shifts, where appropriate and operationally required.

Person Specification

Education/Qualifications

Essential

Registered Nurse or AHP Qualification Registered with a clinical registration body Educated to degree level or relevant equivalent experience

Knowledge

Essential

Significant experience of managing a large multi-disciplinary team of health and ideally social care staff Senior level clinical knowledge in community unplanned/urgent care Possess extensive working knowledge of Primary and Community services Management of change

Budget and resource management skills

Experience of dealing with wide- ranging staff issues, undertaking recruitment and supervision and conducting appraisal interviews for staff. Experience of working with multiple agencies or service providers on a range of patient care issues Ability to lead, manage and motivate staff. Ability to develop and maintain positive working relationships with staff and the wider professional groups or individuals
