Job summary

A series of exciting opportunities have arisen for Band 5 Community Nurses based in Pembrokeshire. We are able to offer a number of locations to best suit candidates across the Pembrokeshire District Nursing Teams in: Pembroke Dock, Haverfordwest, Milford Haven, Tenby, Narberth and Fishguard.

The role will allow you to support the management of a delegated caseload within the Community Nursing Team providing high quality and complex care management to patients in their own homes, with the guidance and support of an experienced District Nurse Team Leader.

The post holder will ensure high standards of care are delivered to patients and their families/carers within the home or clinic environment. This includes liaising with staff, patient/carers and other agencies/organisations as required to ensure appropriate delegation of duties and risk management, whilst ensuring effective communication between those involved in the care package. Pembrokeshire's Community Nursing Teams have a wide-ranging nursing skill mix and pride themselves on supporting professional development and improving high standards of community practice in line with current health policy.

These are permanent positions, worked over a 7-day period including weekends and bank holidays. Full and part time opportunities are available.

The post holder will need the ability to travel between sites in a timely manner.

Main duties of the job

Provide and be accountable for the provision of a high standard of cost effective, safe, individualised nursing care to patients in the community setting through the assessing, planning, implementing and evaluating of patient care. This will be based upon evidencebased practice in line with legal requirements and health board policies relating to practice and the NMC Code.Provide care to adult patients in their own home, residential or nursing care home, clinic setting or holiday accommodation with any medical/ nursing condition.

Have delegated responsibility for assessment, planning, implementation and evaluation of nursing care packages and contribute to the application process for continuing health carefunding.

Deputise for the District Nurse in their absence and support them in the efficient running of the caseload.

Contribute to the induction and training of new staff members, student nurses, pre and post basic students.

Be flexible, adaptable in order to accommodate the needs of the service, be able to contribute to the performance of the community nursing team and develop professionally within the role.

Work alone and with confidence to make decisions without supervision. Work in line with the Health Board Lone Worker policy.

Essential

Level 1 Registered Nurse (Adult) Current NMC Registration Evidence of personal continuing development, statutory and mandatory training Awareness of Health Board policies, procedures and guidelines Ability to relate theory to practice Understanding of primary health care working

Desirable

Post registration experience, in acute services. Teaching and assessing Understanding of clinical governance, health & safety, risk management awareness Community experience Willing to undertake further training as necessary. Evidence of post registration education - Diploma / Degree Level
