Job summary

The Major Trauma Coordinator will work as an integral member of the multidisciplinary After Trauma Team (ATT) to co-ordinate the patient pathway from admission to discharge for patients with highly complex traumatic injuries.

They will work across the hospital to co-ordinate the care pathway of all patients who require on-going inpatient and community rehabilitation following a traumatic injury. This will include co-ordinating the patient's care while in the acute setting and assisting in facilitating the transfer to appropriate interim facilities liaising as required across the Trauma Network and rehabilitation units.

The coordinator will act as a reliable and constant member of the Major Trauma Team and will be expected to contribute to data collection and maintaining patient lists. They will also contribute to the Trauma Follow up clinic and work in close liaison with A&E, ITU, HDU and the wards.

The coordinator will fulfil the role of a keyworker and will be invaluable in liaising with relatives and be a point of contact for patients who have been discharged. There is also the opportunity for practice development and education roles. Close liaison with therapy services and clinical nurse specialists will form part of this.

The successful candidate will have experience of working autonomously and within a multi-disciplinary team, as well as a track record of implementing improvements to patient care through skillful clinical practice.

Main duties of the job

The Major Trauma Centre at The Royal London Hospital is a leading specialist centre with an international reputation for caring for some of the most seriously injured patients across London and the surrounding counties. It is also the home of London's Air Ambulance and the hub of the North East London and Essex Trauma Network, the first and largest Trauma Network in the UK, providing trauma care for more than 5 million people.

Our After Trauma Service is dedicated to ensuring every aspect of care is seamless and coordinated to ensure the best patient experience and optimal long term recovery for our seriously injured patients. The Major Trauma Coordinator will work as an integral member of the multidisciplinary After Trauma team to co-ordinate the patient pathway from admission to discharge for patients with highly complex needs.

They will work across the hospital to co-ordinate the care pathway of all patients who require on-going inpatient and community rehabilitation following a traumatic injury. This will include co-ordinating the patient's care while in the acute setting, liaising with families, neuro navigators and assist in facilitating the transfer to appropriate interim facilities whilst liaising across the trauma networks and rehabilitation units. The position is primarily based at the Royal London Hospital but you may be required to work across the North East London and Essex Trauma Network.

Qualifications

Essential

BSc (Hons) in Nurse / Occupational Therapy/Physiotherapy/ Psychology or any other AHP degree or equivalent State registered (NMR, HCPC or UKCC) Evidence of CPD maintained in a portfolio including attendance at post graduate course/training relevant to trauma specialities

Desirable

Post graduate formal qualification ( MSc Module) Leadership training

Experience

Essential

Relevant post graduate clinical experience, including experience within Major Trauma, Orthopaedics, Neurosciences and complex injury cases Clinical experience in the NHS or similar health service across range of acute/rehab settings

Desirable

Experience in completing Major Trauma Rehabilitation Prescriptions and NMTR data sets

Knowledge

Essential

Understanding of the legal responsibilities of the profession Knowledge of current and emerging NHS and private care rehabilitation strategy and policy
