Main duties of the job

Our Clinical Pharmacist will work as part of the Practiceteam to improve value and outcomes from medicines, as well as consult and treatpatients directly.

The ideal candidate will support the GPs in providing:

Medicines Management and Medicines reconciliation

Proactive Frailty Management

Structured Medication Reviews

In their own dedicated clinics.

In addition, the ideal candidate will work closely with theDispensary Lead in overseeing and supporting the Dispensary team.

About us

Launceston Medical Centre is a busy and committed team whowork together in a recently refurbished and extended Practice., close to thebeautiful beaches of North Cornwall and surrounded by countryside moorland.

Our team consists of 6 GP Partners, 8 Salaried GPs, 6Paramedics, 2 Minor Illness Nurses, 4 Practice Nurses, Trainee Nurse Associateand HCAs, as well as experienced Management, IT, Reception and Administrationsupport. Additional allied professionals who work within our team are ourClinical Psychology team, physiotherapists and Social Prescriber. We are also adispensing Practice, with a experienced team of Pharmacy Technicians, dispensersand Dispensary Clerks. We are also proud to be a training Practice.

We run our own Primary Care Network (Launceston and TamarValley PCN) alongside our neighbourhood Practice, Tamar Valley Health and useour resources for the benefit of the whole community inclusively. Job description

Job responsibilities

Identify patients thatrequire medication reviews and conduct appropriate medication review clinics orhome visits if required by the practice. Liaising with carers,Community Health teams or social care staff to optimise drug therapy, reducepolypharmacy and minimise unnecessary waste, communicating directly withpatients as necessary. Provide medicines andlong-term disease advice and support to GPs and practice staff, registeredpatients, and the wider practice team such as community staff (district nurses,midwives, and physiotherapists) where appropriate. Discuss complex andsensitive information with patients during medication reviews, includingmedical history, lifestyle and personal circumstances and refer to externalservice providers and medical professionals as appropriate. Improve patient and carerunderstanding of, confidence in and compliance with their medications. Help support the safe andeffective operation of the prescribing system in the GP practice, includingupdating repeat medication regimes, issuing of appropriate acute prescriptions,ordering relevant laboratory testing, and if required, when necessary,completing patient referrals (for example audiology, x-ray, diabetes preventionschemes). Provide pharmaceuticalsupport to the GPs to enable them to develop, implement and monitor an agreedprescribing plan which meets the objectives of the medicines management plan. Assist practice and caresettings in developing formularies, guidelines and policies to ensure theimplementation of NICE guidelines, National Service Frameworks and therequirements of the medicines-related aspects of the GMS contract. Advise GPs, care staffand primary health care teams on risk management processes relating to theprescribing and monitoring of specialist drugs controlled drugs and thosesubject to shared care arrangements. Support practice toreview specific areas of prescribing, specified by the line management oridentified with the GP prescribing lead, including the interpretation of e-PACTdata and the use of practice information to review prescribing and identifyareas for action. Assist with thedevelopment and ongoing review of practice policy with regard to repeatprescribing systems and repeat dispensing to encourage the implementation ofprescribing guidance and monitoring guidelines where appropriate. Liaise with CommunityPharmacists to encourage them to reinforce changes made, encouraging them toidentify savings which can be achieved with regard to generics, doseoptimisations etc. and to assist in building their relationship with thepractice. Answer any medicinerelated queries from GPs, care settings, practice staff, PALS, social carestaff and the patients in a timely and appropriate manner, using professionaljudgement where specific guidance is unavailable. Provide advice andsupport to improve the disease registers within each practice to ensure allsuitable patients are identified and offered suitable treatment and monitoringfor their condition. Facilitate andparticipate in multi-disciplinary meetings related to prescribing topics and toengage in the training of other healthcare professionals and practice staff onmatters relating to medicines usage. Attend primary/secondarycare interface meetings in designated specialist therapeutic areas to improvethe cost effective and evidence-based use of medicines across locality anddevelop links with the hospital pharmacists involved in these areas. Undertake any otherduties appropriate to the grade as allocated by line management. Undertake clinicalmedication reviews. QOF & EnhancedServices areas of responsibility to maximise achievement Encourage cost effectprescribing within the practice. Review all safety alerts,ensuring the required actions are taken. Review medication for allnewly registered patients. Person Specification

Experience

Essential

Experience of working within Primary Care. Experience of working within a GP Practice. Experience of using in depth clinical knowledge and understanding of evidence based healthcare. Understand the importance of medicines optimisation in the management of long term conditions.

Experience of using SystmOne.

Skills

Excellent written and verbal communication skills. Ability to work under pressure and meet deadlines. Ability to work independently as well as part of a team. Self-motivated. Flexible and willing to adapt.

Qualifications

Essential

Masters Degree in Pharmacy (MPharm) or equivalent. Independent Prescriber Qualification. Minimum 5 Years post registration experience.
