Job summary

The Adaptations Service is a very busy MDT incorporating Occupational Therapists, Disability Officers, and Disabled Facilities Grants Officers, with NHS and Council staff working together to deliver major adaptations within the community.

Our aim is to help people remain independent by providing suitable adaptations within the home environment, or through carrying out housing needs assessments to provide recommendations to support moving to a property that would be more suitable for their current and long-term needs.

The service covers functional assessments for the environmental needs of both Adults and Children with complex ranges of disabilities and will provide experience with specialities, adaptations, and equipment including major extensions to peoples homes.

The successful candidate will work together with the person and their families, carers, and contracted external companies and building contractors to help prevent unnecessary admissions to hospital / residential settings and allow people to remain in their own homes. This priority is high on the government agenda and is supported by substantial budgets to achieve this goal.

Main duties of the job

Main duties of the job

To be responsible for the delivery of Occupational Therapy within the Adaptations Service, and as a unit of Adults and Health, utilising in-depth theoretical and technical skills to recommend major adaptations to a persons home. To provide professional rehousing reports to highlight the risks within the present environment and the recommendations and priority to move to alternative accommodation. To independently manage a caseload of adults and children with complex needs within the Adaptations Service using evidence based, person centred principles to assess, plan, implement and evaluate interventions. To provide leadership, supervision, and appraisal for junior and non-qualified staff and students. To participate in the planning, development and evaluation of Occupational Therapy Service, holding responsibility for defined projects.

Job description

Job responsibilities

To be responsible for the written recording of up to date, accurate information concerning the Occupational Therapy intervention in line with Trust, College of Occupational Therapy, and Social Services guidelines, and abide by legal requirements governing confidentiality and data protection.

To access and use appropriate electronic databases in order to gather and record specific information, to analysis and establish a care plan and recommendations.

To supervise and review written documentation of non-qualified staff and approve where necessary in case management.

To approve and countersign written documentation completed by students

To be responsible for the completion and collation of appropriate departmental statistical records

To prioritise referrals to the Adaptations Team in accordance with The Care Act.

To evaluate written records and referrals received from other members of the multidisciplinary team, outside agencies and individuals who self-refer.

To provide specialist written reports, discharge summaries, risk assessments and other relevant information and recommendations regarding individual needs as required, in accordance with legal, professional and Trust standards.

To develop resources to assist with the facilitation of team and service development.

To provide Service Users information leaflets and electronic information.

Maintain a professional appearance in line with the Trust and Council dress code

Know the key concepts of the biological, social, psychological and clinical sciences, which are relevant to their profession -specific practice. Awareness of factors that can influence team working.
