Job summary

We are looking for a dynamic and motivated physiotherapist, who is a team player with a positive attitude and commitment to providing rehabilitation in a patient's home. The goal is to optimise patients' independence and wellbeing, enabling them to return to or stay in their own homes following a rapid decline through illness or injury.

Successful post holders must be able to work hours at different locations across Merton. As an organisation we are committed to developing our services in ways that best suits the need of our patients. This may mean a more flexible working pattern is required as per service need, for example we may need to support other teams or cover a 7-day service.

The normal place of work for this post is The Nelson Health Centre, but as a term of employment post holders may be required to work from any of the trust establishments. All flexible working requests will be considered in line with the CLCH flexible working policy.

Main duties of the job

To undertake specialist physiotherapy and holistic assessment of patients in the community autonomously, interpret results appropriately, use clinical reasoning skills and knowledge of evidence based practice, in relation to the individual's clinical and psycho-social issues to determine treatment options, recommending the best course of action.

To be responsible for a clinical caseload working within multi-disciplinary teams, sharing care appropriately ensuring clients and carers receive optimum level of therapy and holistic care within available resources.
