Job summary

This full-time post is an exciting opportunity for an ambitious specialty doctor to join our department in the provision of Paediatric services. There is room in the job plan for applicants to undertake own clinics and this will depend on the experience and ability of the successful candidate.

This is a full-time position but candidates wishing to work part-time or job-share are encouraged to apply. If a person is appointed on a part-time or job-share basis the job plan will be modified following discussion with the appointee.

Main duties of the job

The main duties of this post are located at St Mary's Hospital, Newport. The post holder will perform duties on a shift system with prospective cover for leave.

In all the responsibilities the Specialty Doctor is directly responsible to and under supervision from the on-call consultant or associate specialist.

Paediatrics operates 2 tiers on-call cover. Out of hours the Specialty Doctor is supported by the non-resident on-call consultant or associate specialist.

The Specialty Doctor will be expected to have both neonatal and general paediatric experience and be able to competently manage both sick children and neonates. The doctor will be expected to be able to attend difficult deliveries and to be able to resuscitate a newborn if needed. They will be expected to do routine baby checks and to carry out neonatal ward rounds when on neonatal duty. When on duty for paediatrics the Specialty Doctor will be expected to see, assess and instigate initial investigations and management on all children referred from either the GP or Emergency department, and to look after the children on the children's ward. The Specialty Doctor will also be expected to have up to date paediatric resuscitation skills.

The Specialty Doctor is encouraged to attend and participate in the active professional development programme, which involves clinical meetings as well as formal ward rounds. In addition the Specialty Doctor is encouraged to participate in clinical audit projects.

About us

The Department, integrated within Children's Service's, provides Paediatric and Child Health Services for the Children's ward, Children's day ward, Children's Outpatients (St Mary's and peripheral clinics), Maternity and Neonatal Intensive Care Unit.

Paediatric and child health needs in the community are met through the immunisation programme and nationally agreed health surveillance programmes, with close collaboration with health visitors and paediatric community nurses. There is a comprehensive general paediatric service, also covering paediatric oncology, paediatric diabetes and endocrinology, cystic fibrosis, neurodisability and ADHD.

The post-holder will work within Paediatrics providing medical care for patients There are nineteen paediatric beds. The ward has 12 beds and a designated HDU bed and there is a 6 bed day ward. In practice, approximately half of these beds (including the day ward beds) are used for surgical admissions. Paediatric admissions range from newborn to up to 18 years. There is a separate unit for nursing children with infections or who are immunocompromised. Parents are encouraged to stay with their children. There are excellent facilities on the ward with access to one play therapist.

There is an adjoining office for the junior doctors and visiting students and also the central office for the community children's nurses.

There is secretarial support available within the team, with offices currently on Level C.

Out of hours arrangements and on-call payments

Out of hours the full shift, resident Specialty Doctor is supported by the non-resident on-call consultant or associate specialist.

A paediatric on-call room is available.

Overview of the Paediatric service

To meet the needs of the unfolding National Service Framework for Children, there is increasing collaboration between Health, Social and Education Services. This programme of collaboration across agencies and disciplines will be developed and implemented over the next 5 years. The emphasis of the department is on teamwork with the focus of attention being the child and family.

Day Ward

This is used Monday-Friday 0800-1800 hours and offers facilities for pre and post-surgery, investigative procedures and ambulant care of community patients.

Neonatal Intensive Care Unit

The Unit is situation adjacent to the Maternity Ward and contains 11 cots. There are specifically funded intensive care cots. This unit provides level 2 care and there is good liaison with Portsmouth and Southampton Neonatal Units. The Unit participates in the Wessex Cotline and Central South Coast Neonatal Network.

The David Hide Asthma and Allergy Research Unit.

This purpose built unit has great potential, both locally and nationally, as a research base for allergic disease. Patients can be referred to the Consultant-led outpatient service which is available twice weekly. There are ongoing studies into the development of allergy disease in infancy and childhood.

Job Planning, Continuing Professional Development and Appraisal

The provisional timetable represents a job plan consisting of 12 programmed activities (PAs) per week. The job plan is a prospective agreement that sets out the doctors duties, responsibilities and objectives for the coming year. It covers all aspects of the consultants professional practice including clinical work, teaching, research, education and managerial responsibilities. It will provide a clear schedule of commitments, both internal and external. In addition it will include personal objectives including details of their link to wider service objectives and details of the support required by the consultant to fulfil the job plan and the objectives.

The job plan, for 12 PAs will include 1 PA for Supporting Professional Activity (SPA) in accordance with Terms and Conditions and the Trusts policies and procedures.

Study and professional leave will be granted at the discretion of the department and in accordance with Terms and Conditions, and the Trusts policies and procedures.

The Trust supports the requirements for CPD and is committed to providing time and financial support for these activities. Funds are allocated to each specialty doctor on an annual basis - currently £ 800 per annum.

The successful candidate will be expected to play their part in the postgraduate activities, teaching and clinical audit.

There is an active SAS educational forum led by Mr Ramesh Babu, Associate Medical Director for Education and SAS Tutor.

The post holder will be encouraged to register for continuing professional development (CPD) with the Royal College of Surgeons and the post holder will be expected to undertake activities necessary to fulfil CME/CPD requirements for annual certification and General Medical Council revalidation.

Teaching and Training

The post holder will be expected to play their part in postgraduate activities and teaching. This will include contribution to education and training programmes for hospital medical staff, GPs and other groups of hospital staff.

All educational supervisors of doctors in training will have allocated up to one hour per week per trainee for supervising these doctors and to carry out the responsibilities of educational supervisors as indicated in the Health Education Wessex Postgraduate Deans contract with the Trust and according to GMC guidelines.

The Trust has close links with the Medical School at Southampton, and Final Year students undertake attachments at St Marys.

Job Plan Summary

SPA to be taken during the normal working week and the Trust would expect this to be undertaken on-site but if SPA is time shifted to evenings or weekends this could be undertaken at home with prior agreement of the supervising consultant.

Review:

The ongoing number of PAs and personal job plan objectives will be decided in negotiation with the Care Group Director or Lead Clinician and with the Associate Director of Operations at the three-monthly review of the job plan. At this stage any necessary adjustments will be made to reflect the actual hours worked. Job planning will be conducted annually thereafter.

Mentoring:

The Trust believes that every newly appointed consultant should be offered a mentoring opportunity.

Appraisal:

Post holders will take part in Annual Appraisal and performance review against objectives as well as specific requirements laid out above.

Mr Stephen Parker is the Medical Director and Responsible Officer for the Trust for the purpose of Revalidation.
