Community Reablement Assistant

The Single Point of Access Team are looking for a Full Time (37 hours per week)  Case Worker to join our busy team. We work with individuals aged eighteen plus, this means that our work is varied and changeable and cover individuals with a variety of different needs.

The right candidates will be someone who is passionate about people, has a strengths based and person centred approach and has experience working with adults. 

The successful candidate will be highly motivated and will to undertake Care Act Assessments, support/care planning and commission services with person centred outcomes. They will be able to travel throughout North Lincolnshire independently as we cover all of the North Lincolnshire area. Knowledge of adult social care legislation and frameworks are essential. 
 

You will need to demonstrate an ability to problem solve and good negotiation skills, along with effective communication, organisational and IT skills.
