Serving a population of over 930,000 the Trust's cancer activity is comparable to designated cancer centres, so if you thrive on a challenge and feel you can make a real difference to the care of our cancer patients, this post maybe for you. The Oncology cancer nursing team are looking for a Band 4 cancer clinical support worker to assist them in providing high quality care to patients living with and beyond a diagnosis of cancer. The role will include a mixture of patient contact, management of databases, making appointments and managing our health and well being events. The patient contact will include answering phone enquiries, contacting patients directly, assisting with health & well being events and in filling in forms for both feedback and assessment.

Main duties of the job

The Cancer Clinical Support Worker will form an integral part of the multidisciplinary team approach in caring for the physical, social and psychological needs of patients diagnosed with cancer and their carers throughout the care treatment pathway and after care support.

You will work in close unison with the Clinical Nurse Specialists (CNSs) to ensure the coordination and management of patients at all points of their treatment including stratified follow-up and will be able to signpost patients to support their self-management. You will be responsible for the management of IT systems that support the cancer pathway and be a key contact for patients.

The post holder will work independently within the remit of the role but under the guidance and supervision of the Clinical Nurse Specialists.

The successful candidate will have worked within the NHS and have excellent communication skills, experience in data collection, analysis, and office software and be able to prioritise their workload without direct supervision. Previous experience of supporting cancer patients would be advantageous.

Job responsibilities

Work as a Health/Social Care Assistant to assist with the coordination and management of patients undergoing treatment at LNWHT to support patients both on and off Provide information to patients related to treatment/follow-up and signpost to appropriate healthcare providers within remit of role and Provide telephone advice and triage where appropriate, working within defined protocols and guidelines. Document and communicate outcomes as per service Support stratified Follow- Up for Cancer patients, be responsible for the management of IT systems that support this pathway ensuring that patients are entered and the pathway monitored. Triage telephone calls to the Cancer CNS telephone and liaise directly with patients, signpost and escalate to the CNS team as appropriate. Provide level 1 psychological support within own knowledge boundaries following appropriate Perform Holistic Needs Assessments using designated screening tools as To participate in collating patient surveys and develop action plans alongside the senior CNS. Support CNSs in developing and facilitating Health and Well Being programmes. This will involve contributing and delivering education to patient and carers, including information around health promotion, disease prevention and supporting self-management of their rehabilitation or long term Document all interactions both verbally and written with accuracy and in line with Trust policy and Demonstrate excellent communication skills both verbal and Person Specification

Education/Qualifications

Essential

GCSE/'O' level English and Mathematics NVQ Level 4 in Health or Social care, or equivalent experience/education Evidence of further study, courses related to healthcare or cancer care
