Job summary

Main duties of the job

To provide care for long term conditions such as Asthma, COPD, Diabetes or Hypertension. It is expected that the successful applicant will be responsible for at least one area of long-term condition care.

To offer general practice nurse duties including wound management, cervical cytology, ECGs, dopplers, childhood immunisations, travel vaccinations and immunisationclinics to protect patients against influenza, pneumonia, shingles, covid etc

Job responsibilities

JOB TITLE: Practice Nurse - Chronic Disease

REPORTS TO: NurseManager

ACCOUNTABLE TO: Nursing Lead GP Partner

DUTIES ANDRESPONSIBILITIES

Management of chronic diseases, taking the lead in at least one area ofchronic disease management, to encourage patients to achieve a good quality oflife and to meet QoF requirements.

BASIC NURSINGPROCEDURES

Dressing and wound management. Scalds and burns management. Leg ulcer care and management.

Suture and clip removal.

Minor injuries sprains, bruising, cuts, grazes etc.

Management of skin disorders.

Venepuncture.

Testing pathology samples blood, urine.

Undertake cytology screening.

Ear syringing, blood pressure and weight management.

Aid/chaperone to GP during clinical procedures, minor operations etc.

Specimen collection msu, pregnancy tests, faeces for O&S, sputum, etc.

Putting labels on samples, form filling and ensuring timely preparation of specimens for collection to laboratory.

General observations BP, pulse and temperature.

Use and care of ECG machine

Use and care of the doppler machine

Use and care of nebulisers.

Ensure the maintenance of stock levels in the Treatment Room relating to patient care including medications, vaccinations and dressings.

Respond to medical emergencies
