CLCH Hertfordshire Adult Community Services - CLCH NHS Trust provides a range of Community Services, and the Hertfordshire Division is recruiting

We are currently looking to expand our fantastic nursing team, on the search for motivated, proactive, and enthusiastic individuals who are keen to develop and grow in the world of community nursing.

This post offers experienced nurses the opportunity to develop or expand clinical skills, ranging from Wound care, Palliative care and Iv therapies alongside developing leadership and managerial skills.

There is also the opportunity for successful applicants to explore the Specialist Practitioner District nursing qualification in the future if this isnt something they have already achieved.

Main duties of the job

To provide leadership and management to a team of community nurses To be responsible for the team's assessment, planning, implementation, and evaluation of individualised nursing care to patients living in the community. The service is provided seven days per week 8am - 6pm To be responsible for the team's maintenance of patient and other records To participate in the long-term management of patients within the locality To have continued responsibility for a defined caseload To be committed to teamwork and demonstrate good communication at all times.

Job description

Job responsibilities

To demonstrate discretion and respect when communicating with patients, colleagues and other agencies. To use tact and diplomacy to provide and communicate complex, sensitive information where motivational negotiating empathetic and reassurance skills are needed, where barriers exist to understanding, maintaining confidentiality within Trust Policies. To communicate effectively with all members of Primary Health Care teams. Develop networks for effective communication between health, statutory and voluntary services including patient forums.

Essential

Registered Nurse Mentor/Preceptor/998 or equivalent Specialist Nursing Practice(District Nursing) Degree or will undertake training Nurse Prescribing or will undertake training Knowledge of recent developments in NHS Knowledge of Evidence based practice Evidence of personal development Knowledge of NMC code of professional conduct

Desirable

First Level Degree Relevant courses in Long Term Conditions Management Course of Top to Toe examination/ diagnosis Experience Independent Prescriber
