Main duties of the job

You'll be responsible for:

Leading on and coordinating assessment, planning and evaluation of individualised patient centred care. Ensuring a holistic approach that is sensitive and responsive to the patients ever changing needs. aintaining a respectful non-judgemental and caring attitude ensuring the Rennie Grove Peace Hospice vision and philosophy of care is integral to the care delivered and role undertaken. The coordination and smooth running of a shift ensuring it is well organised and staff and patients are safe. Acting as a Rennie Grove Peace ambassador within the community.

Job responsibilities

To undertake a holistic needs assessment that explores individual patients and their carers/families physical, emotional, spiritual, practical, welfare and if appropriate the families pre and post bereavement needs, using a variety of tools to support the process. To be able to develop, update and monitor the individualised care plans ensuring they are patient centred and support the identified patient needs. To be able to deliver both generalist basic and complex specialist nursing care interventions that is responsive to the individual patients requirements. To be able to communicate highly sensitive and often contentious information, ensuring patients and their families/carers are adequately supported. To be able to demonstrate through documentation, and verbal communication the care undertaken and patient outcomes. To participate in keeping accurate and legible records in accordance with existing practice and procedure at the Peace Hospice. To be aware of the legal requirements associated with keeping such records. To take charge of the Inpatient Unit shift in the absence of the IPU Team Lead. To regularly attend service meetings, team meeting and other intra hospice meetings, ensuring information is cascaded to colleagues accordingly. To demonstrate on going personal and professional development in accordance with yearly appraisal objectives. Be aware of the NMC Code of Professional Conduct and be accountable for own practice, ensuring continued, effective registration with NMC. Person Specification

Qualifications

Essential

Registered NMC Nurse Experience of MDT working IT Literate Effective Communication skills Ability to work autonomously and as part of a team Flexible attitude to working Willingness to take on an extended nursing role Experience of working with clinical audits A commitment to the vision, values and objectives of Rennie Grove Peace

Desirable

Palliative care experience One year post qualification experience
