Job summary

To provide a specialist AdvancedParamedic resource for health care professionals and service users, working incollaboration with other members of the Multidisciplinary Team To work within the community as anautonomous, accountable, Advanced Paramedic Practitioner, in the provision of aholistic approach for individuals including assessment, diagnosis andtreatment, to deliver quality patient services To assess, diagnose, treat, refer orsignpost patients/service users who attend surgery with undifferentiated orundiagnosed conditions relating to minor illness or minor injury The post holder will use advancedclinical skills to provide education to service users, promoting self-care andempower them to make informed choices about their treatment The post holder must own a vehicle forhome visits, for which expenses will be remunerated.

Job responsibilities

Diagnosing and treating patients presenting with minor illness:

Triage and treat patients wishing to seea health care professional, including referral to other healthcare servicesaccording to practice protocols Ensure clinical practice is safe andeffective and remains within boundaries of competence, and to acknowledgelimitations. Advise patients on general health careand minor ailments, with referral to other members of the primary and secondaryhealth care team as necessary and in line with relevant protocols Works from the surgery and withincommunities as an autonomous practitioner caring for patients and families. Works as an autonomous practitioner, inaccordance with the Health Care Professions Council (HCPC). Ensure thatpersonal and professional clinical standards are maintained. To undertake assessment for patientswithin the community and those attending the surgery, using diagnostic skillsand initiation of investigations where appropriate If a prescriber, to prescribe medicationsas appropriate following policy, patient group directives and local pathways. Ifnot a prescriber, to liaise with a GP/prescriber as appropriate To formally and informally impartknowledge and skills to colleagues promoting peer review and best practicewithin the work environment To communicate at all levels within theteam ensuring an effective service is delivered. To maintain accurate,contemporaneous healthcare records appropriate to the consultation. Ensureevidence-based care is delivered at the highest standards ensuring delivery of high-qualitypatient care Work with local policies and procedures Enhance own performance throughContinuous Professional Development, imparting own knowledge and behaviours tomeet the needs of the service To achieve and demonstrate agreedstandards of personal and professional development in order to meet the needsof the service To participate in the audit process,evaluation and implementation plans and practice change in order to meetpatient need Contribute positively to theeffectiveness and efficiency of the team and work colleagues Work in partnership with other APP/GPfor local residential nursing home. Pathological specimens and investigatory procedures: Undertake the collection of pathologicalspecimens including intravenous blood samples, swabs etc. Perform investigatoryprocedures requested by the GPs within competences. Administration and professionalresponsibilities: Participate in the administrative and professional responsibilities of the practice team Ensure accurate and legible notes of allconsultations and treatments are recorded in the patients notes Ensure the clinical computer system iskept up to date, with accurate details recorded and updated Ensure appropriate items of serviceclaims are made accurately, reporting any problems to the practice manager Ensure accurate completion of allnecessary documentation associated with patient health care and registrationwith the practice Ensure collection and maintenance ofstatistical information required for regular and ad hoc reports and audit Attend and participate in practicemeetings as required Restocking and maintenance of practice emergency equipment, and your own clinicalareas. Training and personal development Training requirements will be monitoredby yearly appraisal and will be in accordance with practice requirements.Personal development will be encouraged and supported by the practice. It isthe individuals responsibility to remain up to date with recent developments Participate in the education andtraining of students of all disciplines and the introduction of all members ofthe practice staff where appropriate Maintain continued education byattendance at courses and study days as deemed useful or necessary forprofessional development, ensuring PREP requirements are met If it is necessary to expand the role toinclude additional responsibilities, full training will be given Develop and maintain a Personal LearningPlan Keep up to date with statutory /mandatory training in line with Didcot Health Centre and NHS guidelines. Liaison As well as clinical colleagues, there is aneed to work closely with the reception office to ensure the smooth running ofthe practice, reporting any problems encountered to the relevant person There is also the need to establish andmaintain good liaison with other surgeries and agencies, including secondarycare. Meetings It will be necessary to attend andcontribute to various practice meetings as requested. The only reason for notattending will be annual, study or sick leave. Days off if attending will bereimbursed at standard rate or time off in lieu. They do so in confidence and havethe right to expect that staff will respect their privacy and actappropriately. Qualifications

Essential

A HCP registered paramedic with at least five years experience post qualification. Have achieved Stage 1 of the FCP Roadmap. Own your own car and have a full UK driving licence Commitment to continuing professional development

Desirable

Hold a Certificate in Advanced History Taking and Clinical Assessment. Have an Advanced qualification in minor injuries and illness. Be a non-medical prescriber.
