Job summary

Healthy Crawley PCN are looking for a Paramedic Prescriber tojoin their innovative and dynamic Primary Care Network to focus on supportingthem with their community and housebound patients..

You will work closely with clinicians from the practice andwill play a vital role in increasing capacity for all appointments includinghome visits. The successful candidate will be provided with excellent support,the opportunity for development in the role, and will be clinically supervisedby a General Practitioner.

Main duties of the job

As a Paramedic, you will be required to work autonomously,providing a range of services such as assessment, diagnosis, treatment,telephone triage, clinical decision-making, and referring patientsappropriately under supervision.

You will prescribe andreview medication for therapeutic effectiveness, appropriate to patient needsand in accordance with evidence-based practice and national/practice protocols,within the scope of your practice.

You will be predominantly community based, undertaking homevisits, and liaising with a multidisciplinary team including GPs as well asother services.

You will report to a GP/Partner who will provide support andmentorship.

Alliance for Better Care CIC will provide support and ensureaccess to training programmes as required.

HealthyCrawley Primary Care Network is a NHS Collaboration between four GP Practices Furnace Green Surgery, Ifield Medical Practice, Poundhill Surgery and TheWoodlands/Clerklands Partnership - working together to provide enhanced accessservices.

Our PrimaryCare Network builds on the existing primary care services and enables a greaterprovision of proactive, personalised and more integrated health and socialcare. We are supported by practitioners in additional roles who allow us tocreate bespoke multi-disciplinary teams based on the needs of our localpopulation. By working together with local community services, thisallows us to make support available to people where it is most needed.

Job responsibilities

Clinical Responsibilities

Toprovide assessment, diagnosis, and treatment at first point of contact byattending to patients according to patients needs either in the Practice orpatients homes under supervision. Workwithin a practice in the Primary Care Network, including a combination of inperson, remote, telephone and video consultation. Undertake any tasks consistent with the level of the post and the scopeof the role, ensuring that work is delivered in a timely and effective manner. Work with practice teams under supervision toensure the process for referral of urgent on the day patients is smooth andappropriate patients are referred making full use of the practitioners skills. To accurately triage and prioritise patients,including in emergency situations under supervision, demonstrating the use of avariety of techniques to elicit the history of an event/illness, including pastmedical and drug history. To evaluate clinical information fromexamination and history taking and initiate appropriate treatment and/orreferral. Toundertake and interpret diagnostics and take appropriate action to managepatient care, whilst distinguishing between abnormal and normal findings inorder to relate them to a possible diagnosis. To use broad clinical guidelines (NICE etc) relating them to individualcases, providing assistance and feedback to the organisation and others on howthese should be interpreted. Prescribeand review medication for therapeutic effectiveness, appropriate to patientneeds and in accordance with evidence-based practice and national/practiceprotocols, within the scope of your practice Tosignpost appropriately to other services in the identified healthcare pathway. To refer patients to appropriate care settings, within locally agreedhealthcare pathways. To advise and support members of the public onhealthy lifestyles, health promotion and disease prevention. To maintain accurate and systematicdocumentation of patient interactions and clinical care provided, includingadverse events. Support delivery of anticipatory care plansand lead certain services (, monitoring blood pressure and diabetes risk ofhousebound elderly patients). Recognise opportunities to reduce inequalitiesand unwarranted variation in health care for the practice population. Play anactive role in in-house practice and clinical meetings.

Communication

Ensure via use of clinical systems, attendanceat meetings and other means that are a core part of the practice MDT supportingpatients and work with the wider MDT including staff from SCFT, SPFT, LocalAuthority and Voluntary Sector organisations. Ensure high quality, clear communication withpatients and carers including within challenging situations.

Professional

To proactively support audit of own caseload andthat of colleagues to ensure the quality of the service is maintained andcontinuously improved. To seek out feedback from other members of the MDT andpatients to support this. To undertake all necessary training and befamiliar with all policies to ensure compliance with expected procedures andways of working. To ensure practice is within good clinicalgovernance and based on sound up-to-date clinical evidence. Where appropriate,to identify and assess any risks associated with patient care or to oneself andcommunicate these risks to the appropriate clinical manager. Adhereto organisational policies and procedures, including confidentiality,safeguarding, lone working, information governance, equality, diversity andinclusion training and health and safety. Mentoredby a GP through Level 7 qualification. Participate in the education and training of staff and students ofall disciplines. Maintain continued education by attendance at courses andstudy days as deemed useful or necessary for professional development, ensuringCPD requirements are met. Complete statutory training / updating and mandatorytraining as defined by the PCN. Person Specification

Qualifications

Essential

Registered Paramedic (HCPC) Independent Non-medical prescribing qualification Evidence of continued professional development Has completed 2-year consolidation of learning period as a newly qualified paramedic; and, has a further 3-year experience as a band 6 (or equivalent) paramedic Working at Level 7 capability in paramedic areas or practice (or, aiming to achieve within 6 months of starting) ALS/APLS provider Completed mentorship programme

Desirable

ENP/ECP Minor Injuries and Minor Illness course Level 6 Attended X-ray/IRMER course Teaching and assessing qualification Relevant MSc or BSc

Experience

Essential

Previous experience of non-medical prescribing and using PGDs Experience as an autonomous practitioner within a minor injury/illness centre or mobile service Experience of the ECP/Advanced Practitioner role within Primary or Secondary Care Experience in using IT based patient documentation systems Experience of triage/initial assessment Experience of following referral pathways
