Job summary

This role provides post holders the opportunity to workacross our hospital at home services including our virtual ward, urgent carewrap around service as well as assessing undifferentiated patients. The rolewill provide post holders with opportunities to develop their experience ofurgent and primary care as part of a multidisciplinary team whilst supported byour clinical leads. GPN is an innovative patient focused organisation seekingto support patients to be cared for at home whenever possible.

Main duties of the job

The post holder will be practising in a highly autonomous manner as an advanced clinical practitioner (ACP) This will encompass the skills of history taking, extended assessment, examination, diagnosis and treatment within an agreed extended scope of practice. Providing definitive care without requirement for supervision. Where patients require further input undertake appropriate referrals. The post holder will be a highly experienced independent senior clinician with excellent interpersonal and communication skills and clinical attributes, enabling them to provide expert clinical and professional support to other clinicians and staff

Provide support, advice and supervision of more junior registered and unregistered staff within the service.

Job responsibilities

1. Clinical/patientcare

Todeliver and participate in the clinical care of patients working as a highlyautonomous practitioner.

To ensure that every patientreceives specialised advice and appropriate management to facilitate a safedischarge, transfer or admission as required.

Provide a level ofadvanced and comprehensive assessment based on a specialist body of knowledge,with a focus on assessing and managing risk.

Directresponsibility for the management of patients within own case load andsupervision /development of junior members of the multidisciplinary team.

To appropriatelyassess, examine, investigate, diagnose and treat patients, resulting in thesafe management, appropriate referral or discharge of patients

To gain collateralhistory for patients to ensure appropriate management decisions are taken, thisincludes accessing summary care records

To manage patientexpectations using highly developed communication skills, recognising that manypatients will have chronic conditions that should be managed within primarycare This communication will often involve conflict due to unrealisticexpectations and that managing these expectations will require delivery of acontentious message. To work in an environment that is high pressured, withmultiple competing priorities, caring for patients from a range of backgroundsand conditions.

To contribute tothe development and improvement of systems, policies and processes thatfacilitate patient care.

Maintain ownclinical development by keeping abreast of new treatments and technologieswithin primary, acute and emergency care.

To improve thequality of the patient experience by identifying and meeting the individualclinical needs of patients.

Where appropriateto their presenting circumstances, to rigorously review all aspects of thepatients plan of care and identify and address issues that cannot wait for thepatient to be reviewed by their own GP.

Contribute to thereview and development of integrated care pathways and clinical guidelines toensure a standardised approach.

Practice withconfidence and competence within professional bodies guidelines ( NMC, HCPC).

To ensure thataccurate documentation and records of patient care are kept.

To provide expertadvice on patient care to the healthcare team taking into account all aspectsof clinical governance, and act as a specialist resource within the team.

Ability to adaptexpert clinical knowledge and skills to different clinical settings andinfluence service delivery and patient care.

To prescribe safely andeffectively as a non-medical prescriber in accordance with relevant guidelinesand within the individuals scope of professional competence, to ensure allprescribing practice offers best value for money to the health system.

Actively implementeffective systems and processes for infection control management and relevantaudit within their clinical area.

To be a conscientious,confident and competent practitioner who works within their own scope ofprofessional practice.

2. Managementand Leadership

Support thedelivery of the service KPIs and play an active part in clinical governance,developing and leading on action plans.

Lead and developan area of responsibility related to advanced clinical practice or servicedevelopment.

To co-ordinate andprovide leadership for the non-medical teams.

Deliver the highest level ofinterpersonal and communication skills and be actively involved in thedevelopment of effective communication strategies with other members of themultidisciplinary team. This will include discussions with patients around sensitiveinformation and difficult conversations with patients and relatives.

Participate in thedevelopment of a specialist service and act as a change agent, where necessaryleading staff and facilitating the change process whilst encouraging others tobe innovative and adaptable in the approach to change including development ofnew services.

To act as achampion for the team, proactively liaising with external and internalstakeholders advocating advanced practice provision that is effective andresponsive and that patients and clinicians experience a high standard ofcommunication care and continuity.

To be an effective, competentand confident role-model / clinician and to champion the ACP role to othermembers of the team.

To act as a rolemodel for infection control practice, nutritional standards, medicinesmanagement and falls prevention, ensuring best practice in these areas isintrinsic to care delivery.

To developevidence based policies and procedures specifically required by the team.

To developclinical competencies for staff working within the team.

To develop andmaintain effective communication with the multidisciplinary team to ensure highstandards of care.

To workcollaboratively and strategically with representatives of other clinical areas,disciplines and services to provide a seamless pathway of care.

Participate in therecruitment, selection and retention of staff.

To be conversantwith all policies and procedures and ensure they are correctly implemented.

Participate inbusiness planning and the development of financial bids for servicedevelopment.

3. Educationand Training

To help identify,plan and implement goals for clinical development of staff working within theservice, monitor the development of advanced skills within the team.

To develop andmaintain a positive learning environment for the team by providing support,ensuring developmental requirements are addressed.

To support theprovision of clinical supervision and provide the opportunity to reflect anddiscuss areas of practice.

To plan, implementand participate in teaching programmes for nursing, medical and allied healthprofessional staff of all grades.

To maintain closelinks with local Universities and to participate in the development anddelivery of new and established specialist practice programmes.

4. Auditand Research

Contribute and attimes lead the development of multidisciplinary research and clinical audit inthe service and adjacent assessment areas, implementing effective andevidence-based practice.

Exhibit advancedresearch and audit skills and provide support and appropriate implementationstrategies for changes in clinical practice.

Ensure the resultsof audit and research conducted by the team are disseminated.

To share bestpractice; through publications and attendance/presentation opportunities atconference level.

5. Resource Management

To apply soundfinancial evaluation to proposed service developments.

Manage ownworkload.

Support juniornursing, AHP and medical staff to manage their work load.

Work as a memberof the multidisciplinary team, sharing in the responsibility for the provisionof 7 day, care.

Promote and adhereto the principles and practice of clinical governance.

Responsible on ashift by shift basis for the advanced clinical practitioner management of allpatients within the service.

Demonstrateeffective multidisciplinary team working and acknowledge the role of individualteam members, ensuring that expertise is seen as a resource within the team.

Utilise adeveloped knowledge of vulnerable adult safeguarding procedures.

Support trainee,new and experienced ACPs to manage their workload.

To ensure all prescribingpractice offers best value for money to the health system, that medicationsclassed as general sale and pharmacy items are not provided to patients.

To ensure appropriate use andcare of high value clinical equipment such as portable ultrasound equipment.

6. Risk Management

To ensure thatsystems are in place to manage, reduce and prevent clinical risk to self andothers within acute services.

To assist in theassessment and management of risk in line with the trust standard and alongsidethe operational management team by ensuring adverse incidents are reported,investigated and utilised to develop and open culture of learning from risk.

Share lessonslearned from significant events

To monitorincidents highlighted by members of the team and communicate to appropriateareas to address risk issues.

Promote bestpractice in health & safety, utilising organisational policies.

1.Significant urgent/acute/primary care experience with experience working as a competent proficient fully autonomous practitioner within a relevant environment such as emergency care setting. 2.Established Autonomous Nurse Practitioner/Specialist Paramedic in Primary Care/Unscheduled Care experience of utilising expert diagnostic and cliinical skills to provide autonomous unscheduled medical care and management of patients presenting with undifferentiated and undiagnosed conditions. 4.Specialist knowledge in clinical reasoning , history taking , physical examination , diagnostic skills and treatment. 5.Evidence of ability and knowledge on how to undertake audit and feedback understanding of urgent and emergency care performance targets and ability to identify opportunities to improve service delivery
