This is a fantastic opportunity for someone to join our Community Nurse teams. We are based in a rural/coastal area and cover 5 GP practices with 3 community nurse teams to support them with a range of care aspects and clinical skills for patients delivered in their own home or residential care environment. You need to be able to drive to visit patients in the community. 

You will be provided with a comprehensive induction programme which will include support to complete the National Care Certificate.

Successful candidates will have a Level 3 Diploma / NVQ 3 in Health and Social Care or an equivalent qualification and ideally have experience of working with older people in a community setting. You will also need to be competent with IT as electronic recording is an essential part of the role.

We are proud of the service we provide and we work closely with other members of our multi-disciplinary team including the GP`S, Adult Social Care Team and Urgent Community Response team.

We follow our trusts values and core behaviours at all times. We have regular s and yearly appraisals. We have an e-learning system for updating our mandatory training and ongoing courses.

We work from a computer system called My Care; this ensures that all patient care is documented correctly and patients don’t need to report the same story to everyone. You will also need to be competent with IT as electronic recording is an essential part of the role.
