Main Duties andResponsibilities

Clinical

Telephone consultations of on the day patientrequests, to advise on or signpost for self-care, arrange for face-to-faceconsultations to either see themselves, another member of the clinical team,the Nurse Practitioner, a Practice Nurse, HCA, or to arrange a home visit

To prioritise presenting patients health problemsappropriately in urgent or emergency situations, including initiation ofemergency care if required

Assess, examine, diagnose, plan, implement andevaluate holistic treatment/interventions and care for patients presenting withan undifferentiated diagnosis on an acute or minor illness basis

Provide information and advice to patients inrelation to prescribed or over the counter medications, side effects andinteractions.

Support patients to adopt health promotionstrategies that promote healthy lifestyles and apply principles of self-carewhere appropriate.

Interpret test results such as ECGs, urinalysis,basic laboratory reports and plan clinical care accordingly

To liaise with the practice nurse team forappropriate follow up for patients who require wound management or phlebotomy.

To arrange investigations as appropriate withsecondary care or community services when a need is presented

To follow up patients in the short-term to ensurecontinuity of care

Work collaboratively with other health careprofessionals and disciplines.

Provide a supportive role in helping people tomanage and live with illness.

To discuss with the supervising doctor the needs ofpatients presenting with complex health needs or conditions requiringtreatment, hospital admission or referral

Undertake home visits within the boundaries ofprofessional qualifications and in accordance with the practice Home VisitPolicy

To ensure that any information or advice provided isevidence based and that knowledge and skills are maintained for safe andeffective practice

Participate in the delivery of the Quality andOutcomes Framework, QOF, and support the practice with clinical trialsincluding audit

Administrativeresponsibilities include:

Participate in the administrative and professionalresponsibilities of the Practice team.

Ensure accurate and legible documentation of allconsultations and treatments are recorded in the patients notes.

Use of IT systems such as Accurx to communicate withpatients

Attend and participate in meetings and peer support/ learning as required

Restocking and maintenance of clinical areas andconsulting rooms. Supplies and equipment

Maintenance of equipment and stock relating topatient care

To help cover staff holidays and sickness whenrequired

Training and personaldevelopment

Develop and maintain a Professional Portfolio usingnational guidance such as Health Education Englands Specialist / Advanced LevelPractice guidance as structure.

Maintain requirements for ongoing HPC registrationand, if available in the future, national registration with any otherSpecialist / Advanced level directory of staff (HEE).

Training requirements will be monitored by yearlyappraisal and will be in accordance with Practice requirements. Personaldevelopment will be encouraged and supported by the PCN.

Participate in the education and training ofstudents of all disciplines and the introduction of all members of the Practicestaff where appropriate

Maintain continued education by attendance atcourses and study days as deemed useful or necessary for professionaldevelopment.

If it is necessary to expand the role to includeadditional responsibilities, full training will be given.
