Lincoln, United Kingdom Lincolnshire Community Health Service NHS Trust Full time

Job summary

Lincolnshire Community Heath Service (LCHS) have an opportunity for Registered Nurses to join their Lincoln North Community Nursing Team.

From full time to part time, we have something suitable for everyone's preferences. Student nurses qualifying this year, newly qualified nurses and those with little community nursing experience are also welcome to apply.

As a community staff nurse you will be required to work autonomously and as part of a team, delivering care to patients in their own homes, care homes and clinic settings. Successful candidates will be responsible for assessing care needs, and the planning, delivery and evaluation of nursing care using innovative evidence based practice.

Main duties of the job

Community nursing is exceptionally rewarding as it allows wonderful therapeutic relationships to be developed with patients in their own homes. Working in this capacity embodies the person centred care approach and allows patients to truly be at the centre of care delivery.

The skillset of a community nurse is vast and non-exhaustive requiring knowledge and expertise in a range of areas. Examples of interventions provided are wound management, continence care, palliative care, medication administration and long term condition management. Close working with the MDT (Social Workers, GP's and Allied Health Professionals) to ensure that patients care is effectively co-ordinated, demonstrates a positive impact on our patients.
