Main duties of the job

Responsibilities of this job role

Youwill work as an independent and autonomous practitioner within GP surgeries;this will entail working with no other Physiotherapists at times and willrequire effective communications with the team and wider colleagues.

Youwill work towards Independently performing advanced clinical activities thatfall outside the scope of routine Physiotherapy practice which wouldtraditionally be ordered by Doctors requesting investigations, injections.Understand the relative sensitivity and specificity of particular tests anddiagnostic services such as x-rays, scans and blood tests.

Youwill be expected to identify the most appropriate pathway of care for thepatient including when referral to secondary care is indicated. Referring patients who require on-goingphysiotherapy/rehabilitation to the most appropriate service.

You will provide high quality responsive care foradults and children with musculoskeletal and orthopaedic conditions and provideclinical leadership to Physiotherapy colleagues, other Allied Healthprofessionals and medical practitioners.

Job description

Job responsibilities

To manage clinical, non-clinical andprofessional risk relating to own scope of extended professional practice asguided by the Extended Scope Professional Network (ESPN) and service level competenciesand protocols.

To work within the PSN, professional(CSP, HCP) clinical guidelines, IRMER regulations, professional standards,rules of professional conduct and clinical guidance and apply knowledge ofnational and local requirements for monitoring quality of practice.

To act as a highly specialised source ofexpertise in the assessment and management of neuromusculoskeletal conditionsand to provide spontaneous advice and planned teaching and instruction to GPs,GPwSI, Medical Consultants, Orthopaedic Trainees, Physiotherapy colleagues andother Allied Health care Professionals (AHPs).

To work as an autonomous specialisedphysiotherapist within the Outpatient Physiotherapy Department. The Practitioner will provide high qualityresponsive care for adults and children with complex musculoskeletal,orthopaedic and rheumatological conditions, through the delivery of expertmanual therapy and exercise skills and use of expert knowledge in patientself-management and education.

To work with and have knowledge of thewider multi-disciplinary team, including clinical Psychologists and Consultantsas well as services external to the trust which are regularly referred to, toensure a seamless delivery of care.

To have detailed knowledge of widerhealth, healthcare and social care agencies related to specialities ofPhysiotherapy, Orthopaedics and Pain.

Organisational

To work both as a specialistPhysiotherapist in the Physiotherapy Department and as an independentautonomous practitioner in areas of isolation across multiple GP sites in the Eastof Berkshire without immediate support. Emergencyor urgent clinical assistance can be summoned using a nominated GP within thatsurgery.

To be responsible and professionally andlegally accountable for own highly specialised and complex clinical FCP andPhysiotherapy work load and maintain a high standard of clinical care forpatients in accordance with national standards, nationally published guidelineswhere they exist, or national peer review.

To give assurance of clinical competencyand quality assurance when working outside the scope of physiotherapy byreceiving clinical supervision and mentorship from a nominated GP withspecialist interest in Musculoskeletal supported by the Clinical specialists andpeers.

To be an active member of the MSKPhysiotherapy team in service training programme by attending and deliveringadvanced level presentations and training in team meetings, training sessionsand by attending external courses.

To prioritise and independently managethe clinical and non-clinical components of the job within set time frames asdefined by Trust, service and CSP guidelines patient notes to be recordedwithin 24hr of patient assessment anddaily radiology and mentoring records.

To supervise, coach and provide training,both theoretical and practical, to MSK Physiotherapists in the Physiotherapy Out-patientDepartment, feeding back to the physiotherapy Team Lead as appropriate.

To provide highly specialised MSKclinical leadership through education and training to staff outside of theimmediate service and Trust: Physiotherapists, GPs, GPwSIs, trainingOrthopaedic Surgeons, other medical practitioners and AHPs. This may involve PRof the FCP role, including education on pathways and processes.

To adhere to Trust and service Policiesand procedures at all times.

To take responsibility for implementingprotocols, procedures and guidelines within the specialist clinical areas ordering radiology investigations, red flag and sinister pathologyprotocol

To deputise for the service manager inoperational issues when required.

To project a professional profile ofPhysiotherapy and musculoskeletal medicine at all times and represent andpromote the FCP role and the MSK Physiotherapy service in multidisciplinaryteam, trust and commissioning based settings.

To contribute to the development andmaintenance of the Trust and service objectives, goals and philosophy.

To feedback appropriately to the servicemanager and Physiotherapy Team Leads on issues that affect the provision ofcare or other duties affecting service delivery.

To take the lead for a delegatedspecified area of professional practice within the service (ClinicalGovernance, CPD, Risk management) and co-ordinate and develop this work bothwithin the trust and beyond, including interagency working.

To work with the Service Manager toidentify, understand and implement national strategy for healthcare deliveryand monitor and evaluate its impact on service delivery.

To work with the Service Manager to participateand assist in the process of ensuring growth of this role complement theexisting service help the service manager to develop, set up and integratethe role with the referral management into MSK Physiotherapy and beyond.

To be responsible and accountable forthe actions and quality of work, both own and those staff whose responsibilityis delegated to you such as more junior Physiotherapists, clinical assistantsand administrators. This will include the use of formal appraisal processes.

To be aware of health and safety aspectsof your work and implement any policies which may be required to improve thesafety of your area, report untoward incidents.

To be responsible for the use ofequipment during Physiotherapy duties and to adhere to Department, Trust, localand national policies including competence to use equipment and to ensure thesafe use of equipment by others through supervision, training and teaching.

To contribute to the FCP andPhysiotherapy Service audits, research and evaluation by ensuring clinicaleffectiveness of, and evidence base for, the management of musculoskeletalconditions. Working in conjunction withthe service lead, areas of service development will be identified andundertaken in order to develop best practice and a seamless service forpatients.

To be professionally and legallyaccountable for all aspects of own clinical practice through maintainingstatutory body registration and adherence to professional standards (HCPCguidelines) relating to code of conduct, confidentiality, accountability andscope of professional practice.

To maintain and develop further knowledgeand skills required to practice at an advance level within the scope ofPhysiotherapy and also to use an extended scope of practice beyondPhysiotherapy.

To keep up to date with anydevelopments, bringing them to the attention of the service Manager and widerteams and incorporate new trends, guidelines and practices into everyday work NICE back pain and sciatica guidelines

To recognise and act upon potentialareas of conflict of difficulties; to know channels to refer to, techniques toresolve issues when appropriate and report to the Service Manager orPhysiotherapy Team Lead where necessary.
