An exciting opportunity has arisen to join South Tees Hospitals NHS Foundation Trust.
We are looking for enthusiastic individuals with good interpersonal skills and a passion to deliver a high standard of care to patients at risk of liver cancer.
You will be working alongside established teams of liver clinicians, specialist nurses, allied health professionals, and MDT and cancer care co-ordinators to provide care for patients.
We are inviting applications from highly motivated individuals with excellent communication and keyboard skills.

You will be expected to work at both of our acute hospital sites in Middlesbrough and Northallerton as required and be adaptable & flexible in your approach.

Main duties of the job
The role will involve co-ordinating care from referral to treatment. You will have regular contact with patients, families, and carers to support and signpost to ensure effective progress along care pathways, in liaison with other colleagues and departments in the hospital and the community.
The role will involve supporting high quality personalised care including:
Co-ordinating and monitoring of diagnostic pathways to increase rates of uptake of diagnostic tests Carrying out holistic needs assessments and care plans Preparing treatment summaries Offering health and wellbeing advice and supportYou will be expected to attend the complex hepatology multi-disciplinary team meetings as well as with wider multi-disciplinary team meetings where required to transcribe MDT outcomes using NHS IT recording systems. Development and co-ordination of a hepatology database to increase identification of patients at high risk of liver cancer. Monitoring of patients referred for 6-month ultrasound surveillance to increase rates of uptake for patients invited.

Knowledge & Experience
Essential
Significant NHS experience or equivalent Experience working with a multi-professional team Experience of direct patient contact including telephone and face to face contact.

Qualifications

NVQ Level 3 or above in business administration or equivalent relevant qualification A demonstrably high level of numeracy, written and spoken English. (GCSE grade C and above or equivalents)
