Job summary

Leeds Student Medical Practice and The Light SurgeryPrimary Care Network Limited (LSMP & TLS PCN) are looking to recruit an enthusiastic and forward-thinking Clinical Pharmacist, preferably who has previous PCN Clinical Pharmacist experience.

The role will involve working alongside our Lead GP, senior clinical pharmacist, clinical pharmacists and pharmacy technician. This will be anexciting opportunity for the successful candidates to join our team and helpinfluence and shape the future direction of the team. Direct patient careis a large part of this role and patient reviews will be carried out alongsidethe team of PCN Clinical Pharmacists. Itis essential for the successful candidate to have experience of servicedevelopment and experience in working in Primary Care.

The PCN comprises of two Practices; both arecommitted to creating a supportive working environment where the role willdevelop over time. The successful candidate will be supported to complete theCPPE pathway with dedicated training and support from GPs and the Senior ClinicalPharmacist.

Main duties of the job

The Clinical Pharmacist should be able to workautonomously and be able to provide support to our patients to manage theirlong-term conditions effectively, through complex polypharmacy medicationreviews. In addition to playing a pivotal role in our vaccinationprogrammes.

The patient population across the PCN ispredominantly students and young working professionals; therefore, it isessential that the successful candidate will be able to demonstrate interest inhelping young people manage their medication and also have a proactive view ondisease prevention. Experience or an interest in Mental Health management andprescribing is desirable as the patient population based across the PCN has alarge percentage of mental health patients.

The successful candidate should be able to provideadvice and guidance to practice staff and patients with regards to prescriptionand medication queries, policies, procedures, PCN DES, QOF, local incentiveschemes, audits and medicines safety.

Our PCN does not have any care homes attached toour footprint.

Job responsibilities

The post holder will work closely with LSMP &TLS PCN Pharmacy Team under the supervision of the Senior Clinical Pharmacistand PCN Clinical Director to support the PCN with regards to prescription andmedication queries. Theclinical pharmacist will contribute to the implementation and delivery of the PrimaryCare Network Contract Directed Enhanced Service (DES).

They will provide clinical support to thepharmacy technician and clinicians and support with queries around medication,acute prescription requests and medicines reconciliation. You will helppatients to get the best from their medicines by proactively identifying unmetpharmaceutical need using risk stratification tools, provision of predominantlylevel 3 medication reviews, supporting self-care, minimising clinical risk,answering complex medicines queries, and aiming to reduce wasted medicines.

The post holderwill be responsible for encouraging the development of better understanding ofthe principles of medicines optimisation throughout the practice teams and withother stakeholders and promoting good practice in line with therapeuticdevelopments. This will involve assisting the PCN in achieving nationalrequirements, NICE implementation, and utilisation of medicines optimisationinitiatives.

The post holder will be undertaking structuredmedication reviews, improving medicine optimisation and safety, as well asrunning practice clinics. To provide professional specialist medicines adviceto health care professionals associated with the PCN to promote the safe,effective, and high quality, cost-effective use of medicines in all prescribingdecisions for the benefit of the Practice's patients. You will also beactioning acute prescription requests and conducting medicines reconciliation.

Qualifications

Essential

Masters degree in pharmacy (MPharm) or equivalent Registration with General Pharmaceutical Council up to date

Desirable

Completion of relevant CPPE Primary Care Pathways

Experience

Essential

Demonstrable experience as an established foundation-level pharmacist, demonstrated within a practice portfolio. Experience of partnership/collaborative working and of building relationships across a variety of organisations. In depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare

Desirable

An appreciation of nature of GPs and general practice Previous PCN Clinical Pharmacist Experience.

Skills

Essential

Ability to work and set targets within healthcare. An appreciation of the nature or primary care prescribing, concepts of rational prescribing strategies for improving prescribing. Understanding of the wider determinants of health, including social economic, and environmental factors and their impact on communities. Excellent interpersonal, influencing and negotiating skills. Excellent written and verbal communication skills. Good knowledge of IT systems, including ability to use word processing skills, emails, and the internet to create simple plans and reports. Able to obtain and analyse complex technical information.
