Job summary

A great opportunity to join anestablished Older Adult Mental Health Team who deliver services to the Worcesterand Droitwich localities. The core working hours for theteam are Monday to Friday 9-5. There isthe opportunity for flexible working within this role around the core hours.This role easily allows you to blend working from home and working in theoffice.

We are a supportive team who regularlywelcome students. The service has astrong focus on person centred care and understanding the expression of unmetneeds in the person with Dementia. Thereare opportunities to develop specialist skills and you will receive regularsupervision.

Main duties of the job

This post is a band 6 CMHN. You will have a caseload of patients who havebeen referred to the service for a diagnosis of Dementia or support with aMental Health condition. Also, you will see patients who already havea diagnosis of Dementia and have a more complex presentation requiringspecialist advice and treatment. Youwill manage the patients care from assessment to discharge.

It will be beneficial to have good timemanagement skills as you will manage your own diary. You will attend MDTmeetings weekly to discuss allocations and complex cases

You will develop the skills to undertake holistic assessment ofpatients and develop individual care plans and risk assessments with thepatient.

Job description

Job responsibilities

Act as a rolemodel to colleagues, always seeking to maintain the highest standards ofprofessionalism

Understand andapply person centred care at every stage of contact with the patient.

Work as part of the Multidisciplinary teamacross a range of clinical pathways

Manage a caseload of patients withcomplex Dementia needs, using evidence based / patient centred principles toassess, plan, implement and evaluate interventions.

Maintain robustcommunication with patients, carers and other local agencies. Maintain accurate timely clinical records.

Support patientsand carers to achieve agreed outcomes and goals utilising the recovery model.

Provideleadership for junior staff, through supervision and appraisal.
