Job summary

Are you looking for a new beginning? Do you enjoy working with adults? Our Wharfedale Recovery Hub provides assessment and rehabilitation for patients' ongoing needs following a hospital stay to enable them to regain independence to return home at the earliest opportunity. As a healthcare support worker you will work alongside a team of professionals to help patients achieve their recovery goals. The hub is a 24/7 service and various day and night shifts need covering. A induction period will be provided along with relevant training and support to develop.

Main duties of the job

Thepost holder will be providing direct and indirect patient care independently.The post holder will work within a range of procedures and pathways in whichthey have received training to be competent these include, nut not limited to:activities of daily living, simple wound care, observations, moving andhandling of patients and equipment, working as part of an MDT and supportingstaff.

Job responsibilities

Job Purpose

The post holder will be providingdirect and indirect patient care with the support and supervision availablefrom the staff who are responsible for the care of patients within the clinicalarea. In doing so, they will support the assessment of patients andimplementation of programmes of care delegated to them by the appropriate staffmember ensuring that they practice within their sphere of competence andknowledge. The post holder will work within a range of procedures and pathwaysin which they have received training in order to be competent.

Key Responsibilities

1. Clinical

The post holder will deliver care to patientswithin the limits of the procedures and pathways in which they have achievedcompetence.

Support and assist other staff members todeliver care as delegated and directed

Demonstrates competent physical skills obtainedthrough practice use of moving and handling equipment, routine testswithin agreed pathways of care

Able to identify risk or changes in conditionand are responsible for taking action and alerting the appropriate staff member

Exchanges factual information with patientsusing persuasion, reassurance, tact and empathy and addressing barriers tounderstanding in order to engage the individual in their care and gain validconsent; records discussions accurately and in a timely manner in theindividuals patient record

2. Leadership

Ensures that a professional service and image ismaintained at all time, thereby acting as a role model to all staff

Supports a leadership style which is underpinnedby strongly held values around equality, diversity and openness; effectivelybuilds and maintains relationships with team members.
