Are you a highly motivated and dynamic Qualified Nurse or SCPHN with significant Children and Young People experience wanting to join an 'Award Winning' team? Do you have a specialist interest in providing the best care for Children Looked After (CLA) and a passion for emotional and mental health? Do you possess the skills to develop and maintain a sustainable role for CLA mental and emotional health within our team?
We are seeking to recruit a permanent part or full time ( to hours/week) Band 7 Specialist CLA Nurse with extensive mental and emotional health experience to join our friendly and supportive team. You will be required to undertake complex statutory health assessments, attend safeguarding meetings and support processes within our team to enable our CLA cohort to reach their full potential.
Time management skills are essential alongside strong interpersonal and communication skills demonstrating an empathetic and compassionate approach.
You will have an opportunity to negotiate a flexible and agile working pattern, alongside working in SFT settings and from home with working hours between Monday and Friday 9-5. You will have a thorough induction period and will be supported to develop this role and to further your development within your role. You must be resilient, enthusiastic, and committed to team and multi-disciplinary working.
Main duties of the job
Work alongside the Named Nurse for Children Looked After (CLA) and on behalf of the Trust to improve the recognition, assessment, co-ordination, access, and delivery of health care for Children Looked After, with a specific focus on those with additional mental and emotional health needs.
Undertake statutory health assessments for Children Looked After specifically those with known additional mental and emotional health needs.
Work collaboratively with carers and multi-agency partners to progress health assessments, health care plans and enable a good understanding of the mental and emotional health care needs of the young people in their care.
Provide specialist advice and consultation on the care and treatment of Care Leavers to carers and the wider partner agencies, ensuring a seamless transition into adult mental health services where this is indicated
Work collaboration with the Named Nurse CLA, other members of the Children Looked After Health Service and CAMHS to contribute to and develop programmes/pathways/policies and processes which support this cohort of children to make healthy lifestyle choices and optimise their health
Provide specialist expertise in relation to the emotional and mental health needs of Children Looked After and Care Leavers to the CLA Health Service, staff across the Trust and the wider Integrated Care System
