Associate Band 6 Clinical Nurse Specialist in Palliative Care discharge planning

An exciting opportunity has arisen for a band 6 registered nurse to work within the palliative care team. The suitable candidate will also get an opportunity to develop their wider skills in palliative care through the holistic assessment approach, encompassing physical, social, psychological and spiritual needs. 

The post will support the Macmillan Colorectal CNS to provide a consistently high standard, evidence based service to patients diagnoses with colorectal cancer. 

The Colorectal service at North Middlesex University Hospital Trust provides diagnostic and treatment services for benign and malignant colorectal conditions. Diagnostics, surgery chemotherapy and radiotherapy are carried out on the NMUH hospital site.

The post is a permanent post.

Main duties of the job

To support the Macmillan Colorectal CNS to provide a consistently high standard, evidence-based service to patients diagnosed with colorectal cancer. This support extends to family / carers. To take a key role in the CRC MDT meeting, preparing the cases for presentation, ensuring the minimum data set is completed. The following up post MDT to ensure the duties required post MDT are done by the supporting SHO. To provide expert advice and education to the multi-disciplinary team, patients and carers on all aspects of care for the individual who has CRC, including surgical options, adjuvant therapy and living with the physical impact of the disease. Meet with the Macmillan Colorectal CNS on a daily basis to discuss work and activities for that day, and management plan for patients.  Meet on a weekly basis with the CNS team covering Stoma Care, ERAS and the office admin to discuss mutual patents, address any concerns and develop a management plan for patients.  Encourage patients to take responsibility for their own care, follow up and signpost to the appropriate departments/ agencies as needed.  Undertake the Macmillan HNA for all new patients and at agreed points in the pathway.

Duties and Responsibilities

• To support the Macmillan Colorectal CNS to provide a consistently high standard, evidence-based service to patients diagnosed with colorectal cancer. This support extends to family / carers.

Demonstrates leadership through clinical expertise, delivering high standards of person-centered care and using the underpinning philosophy of co-creating care with people affected by cancer. Actively promote the speciality when seeing patient in wards or outpatients, liaising with the staff in the department. Explaining the role and the input to the patient Documentation of interactions, ether in Care Flow or Somerset, in accordance with NMC Professional Standards Encourage patients to be empowered to take responsibility for their own care, follow up and signpost to the appropriate departments/ agencies as needed. Report any clinical matters/ concerns arising to the Macmillan Colorectal CNS Participates in London cancer Alliance, Colorectal meetings.

Education and qualifications

Essential criteria

First level RGN First level degree in related subject (Cancer, Palliative Care or lower GI) or currently undertaking a Degree Pathway. Post-basic qualification in cancer/palliative care or relevant subject or working towards this. Evidence of continued professional development Advanced Communications Skills Training

Desirable criteria

Research qualification Recognised teaching qualification or prepared to work towards Evidence of Masters level learning or prepared to work towards Physical assessment skills training
