Health Champions have an important role to play in helping people in their community to make positive changes to their health and wellbeing by encouraging others to make healthy lifestyle choices. Enfield Unity Primary Care Network are looking for an experienced Health Champion to coordinate our health inequalities work and support a network of Health Champions across the Enfield voluntary sector.

Networking- Develop the local Health Champions network, support them in achieving the PCNs reducing health inequalities aims and objectives.

Inequalities in healthUnderstand local health inequalities and help to develop policies and strategies for addressing these

Main duties of the job

Our Health Champion Coordinator will have an understandingof all areas of health improvement:

Physical activity Signposting people to gyms, community and online exercise classes

Healthy eating Such as introducing people to healthycookery classes in their area or encouraging their workplace to provide morefruit and vegetables for employees

Heart Health - Support a healthylifestyle to encourage better cardiovascular health including blood pressureand pulse checks

Cancer Screening -Explaining, promoting and myth busting concerns around screening programmes.

Vaccination & Immunisations -Explaining, promoting and myth busting concerns around vaccinations andImmunisations.

Mental health support Direct people to mental health support services and work with people on theirmental resilience through practices like mindfulness

Signposting to local services Develop an in-depth knowledge of services to direct people to such as sexualhealth support, smoking cessation resources or drop-in centres.

About us

The successful applicants will be based over one or two of Enfield Unity PCN's 17 sites with regular outreach to local community services.

Medicus Health Partners (MHP) is the lead practice for Unity PCN and will be the employing organisation.

The successful applicant will have access to the training, support, and governance systems in place at MHP.

Applications are welcome from those with existing primary care or GP practice experience, or applicants that have a keen interest to develop clinical experience in primary care.

You will join a dynamic multi disciplanary team, helping to make a real difference to peoples lives by empowering them to take control of their health and wellbeing, using a holistic approach and connecting people to community groups and statutory services for practical and emotional support.

Main Duties

Networking - Develop the local Health Champions network, support them in achieving the PCNs reducing health inequalities aims and objectives.

Effective communication - Support health messages and encourage local Health Champions to reach their community to make positive changes to their health & wellbeing.

Organise Health Champion training, meetings and health promotion events in the community.

Specific Duties The duties and responsibilities may include any or all of the items in the following list.

1. Build and maintain a database of local Health Champions in the community and voluntary organisations.

2. Coordinate Health Champions network meetings, training and events.

3. Provide support, information, training and guidance to Health Champions on the various health promotion areas of focus as directed by the PCN Inequalities Lead.

4. Attend community venues and events to promote various health promotion activities.

5. Co-design health improvement programmes, materials and training.

6. Conduct blood pressure and pulse checks in the community, provide health lifestyle advise, signpost to local health and social prescribing services.

7. Utilise population health intelligence to proactively identify areas of inequality and work with a cohort of patients with long term health conditions to deliver personalised care.

8. Support patients to utilise decision aids in preparation for a shared decision making conversation.

9. Support people to take up training and employment, and to access appropriate benefits where eligible.

10. Support people to understand their level of knowledge, skills and confidence when engaging with their health and wellbeing.

11. Assist people to access self-management education courses, peer support or interventions that support them in their health and wellbeing and increase their activation level.

12. To be aware of the PCN philosophy and to treat people with dignity and respect, offer constructive support to colleagues and the general public.

13. To be responsible with other members of the team for maintaining and monitoring the PNC & practice systems

14. Taking responsibility for own development, learning and performance and demonstrating skills and activities to others who are undertaking similar work.

15. Inform the lead GP of any concerns regarding the role and any professional development needed.

16. Be aware of own professional boundaries and what to do when you have reached them.

17. Contribute to the effectiveness of the team by reflecting on own and team activities and making suggestions on ways to improve and enhance the teams performance.

18. Promoting and maintaining their own and others health, safety and security as defined in the PCN Health & Safety Policy.

19. Computer data entry/data allocation and collation; processing and recording information in accordance with procedures

20. Providing clerical assistance to PCN staff as required from time to time, including word/data processing, filing, photocopying and scanning.

21. Initiating contact with and responding to requests from patients, other team member and associated healthcare agencies and providers

22. To deal with delegated tasks efficiently, according to guidelines set, and reporting regularly any problems and targets reached as required.

23. To be aware of all procedures and work in accordance with written protocols

24. Understand the principles of shared decision making and work in a person-centred way

25. Understand and maintain infection control procedures

26. Understand and work within GDPR and good practice guidelines for information handling, including patient confidentiality.

27. Work flexibly according to the needs of the service and provide cover for absent colleagues.

28. Assist and supervise more junior members of staff to ensure effective and safe practice.

29. Attend PCN and other meetings as required, both on-line and face to face.

30. Attend other sites as directed to carry out any tasks or services relevant to this role.

31. Undertake mandatory and additional training as identified by the PCN.

32. Any other duties, appropriate to the grade, as required by the service manager or Lead Clinician.

33. Duties may be varied from time to time under the direction of the Lead Clinician/ service Manager, dependent on current and evolving workload and staffing levels

Person Specification

Qualifications

Essential

Recognised professional qualification in social work, health, education or equivalent, and evidence of a commitment to continuing learning and professional development. Qualification in motivational coaching and interviewing. Certification in Safeguarding. Training as set out by the Personalised Care Institute and shared decision making.

Desirable

Educated to degree level. Level 2 Award in Understanding Health Improvement or health coaching equivalent. Experience with EMIS clinical systems. Previous experience working in primary care, the NHS, or Social Care. Experience working with people with learning disabilities and/or serious mental illness.

Experience

Essential

Experienced in Primary Care, Community Care, Social Work or Public Health with demonstrable experience in Coordinating Teams. Strong organisational skills. Ability to listen and communicate effectively with a wide range of people. Motivated by helping people, and be caring and empathetic, and committed to providing a high-quality service. Flexible in approach to adapt to the changing needs of the service. Ability to develop and maintain effective working relationships both within and outside the PCN. Acts in a non-judgmental and non-discriminatory way at all times. Understands the importance of patient confidentiality. Organized. Motivated. Ability to work under pressure. Ability and enthusiasm to work creatively and independently with an appropriate amount of supervision and as part of wider teams. IT Skills (MS Office) and Outlook email and experience of using database software. Availability to work some evenings and weekends in community settings, to work flexibly as required by the needs of the service and willingness to travel within the Enfield area.
