Unplanned Service - Advanced Practitioner (Band-8a)

Are you an Advanced Practitioner looking for an opportunity to work within a dynamic community team?

The role includes providing enhanced nursing, and social and clinical assessment of housebound patients with escalating complex needs, within a robust MDT structure.

The primary function of the role is to maximise the patient's health, clinically assess and treat patients in defined groups thereby reducing unnecessary admissions to acute services, facilitating the delivery of effective, co-ordinated, and timely high-quality care to patients.

Main duties of the job

As a member of the community unplanned team working as an advanced practitioner(AP) in the community setting. Work collaboratively in providing flexible, dynamic and innovative care, responsive to unplanned health needs, focusing on those service users who present with complex needs, co-morbidities and long-term conditions. To proactively manage a time limited episode of care of very high intensity users of services, for treatment and onward referral to enable the patient to maintain independence, avoid unnecessary hospital admission and facilitate early discharge in coloration with Virtual ward pathways. To seek and develop innovative and pioneering ways of working, challenging decisions made by health care professionals. To establish close working relationships with staff in primary, secondary, intermediate care, social care and voluntary organisations.

Qualifications

Essential

Current NMC Registration/relevant allied health care professional qualification Advanced Practitioner Qualification/Masters' degree in relevant study

Desirable

Community Nurse Qualification Qualification in the management of one or more chronic diseases/long-term conditions

Experience

Essential

Minimum 5 years' post registration and 2 years working at band 7 or above Experience of care co-ordination/care management.

Desirable

Experience of working in community care/primary care setting
