Job summary

Lander Medical Practice is seeking a motivated and experienced clinician to join our forward thinking multi-disciplinary team as a Long-Term Condition Practice Nurse. The hours are negotiable and flexible.

Lander Medical Practice is located in the city of Truro and was recently rated in the top 10% of Cornish practices. We serve a population of over 20,000 patients and we are looking for an experienced Nurse to contribute to the practice and to develop their professional career. The post holder will have at least two years post-registration experience in managing long term conditions.

Main duties of the job

We are looking for an experienced and caring professional to provide a high standard of evidence-based nursing care for our patients with long-term conditions including asthma, COPD, Diabetes and Cardiovascular disease. The successful candidate may have experience and qualifications in undertaking IUCD/implants, although this is not an essential skill.

Working both individually and together with other members of the team, the duties will include all tasks normally undertaken by an experienced RGN and in addition to any further roles agreed between the nurse and GPs, having regard to current training.

About us

Lander Medical Practice presently has team of dedicated GPs, nurses, physiotherapists, HCA's and many ARRS roles employees. Together with our neighbouring practice, we form the top rated PCN in Cornwall. We have excellent links with the community nurses, acute care at home team and a pharmacy which are all in the same building. We are located within Truro Health Park and also have a satellite surgery based in Threemilestone.

We have a long history of GP training and are also part of the Exeter Medical School programme. We also provide placements for nursing students from Plymouth University. Every month we have a clinical meeting and allocate protected time for practice development in away days.

Person Specification

Knowledge

Essential

Knowledge of managing patients with long term conditions including Diabetes, Asthma and COPD

Experience

Essential

Two years post-registration experience in Long Term Condition management at a Band 5 or above Experience of working within a GP practice High level of clinical skills including cervical cytology, family planning/woman's health, immunisation and wound management Excellent communication skills, both written and verbal Good IT skills

Desirable

Working as a band 6 in general practice

Qualifications

Essential

Nursing degree qualification Registration with the NMC Qualifications in Diabetes, Asthma and COPD

Desirable

A family planning qualification Qualification in inserting implanon and IUCD
