Cornwall Partnership NHS Foundation Trust (CFT) is excited to offer opportunities for 2 qualified Nursing Associates within our Community Nursing team in Truro.

Are you passionate about providing high quality patient care and would like to be part of the Truro District Nursing team.

You should possess commitment to team working and innovative delivery of excellent standards of care. You will need to be able to demonstrate a real enthusiasm for community nursing as well as a desire for on-going professional development.

You should also have an understanding of the changing climate of care delivery in the community with the ability to travel independently across the area to fulfil the requirements being essential.

Full and part time hours available.

**Main duties of the job**:
The role of the Nursing Associate is to work with the Registered Community Nurses and other members of the multi-professional team, providing and monitoring care and contributing to the ongoing assessment, as agreed with supervising clinician(s) and in line with the service pathways. They will monitor the condition and health needs of people within their care on a continual basis in partnership with the multi professional team.
