**Do you enjoy crisis style work but not the shift work?**

We are looking for an experienced individual to support service users requiring increased support from the Community Mental Health Team (CMHT). The team has a strong group of care co-ordinators, however there are times when service users require more input than a care co-ordinator can provide. At these times we utilise 'shared care'. We are looking for an individual to lead on the 'shared care' work. This is pre-planned work supporting service users who may have increased risk, emotional distress or relapse in their mental illness. As these are the service users we are most concerned about, there is an expectation of joint working with the Crisis Resolution Home Treatment and Mental Health Liaison Teams.

The Winchester Adult CMHT is looking for an enthusiastic, dedicated, and talented professional to join the team. Applicants will need to hold a full UK driving licence (or equivalent) and have access to a suitable vehicle for work.

Main duties of the job

To be an active member of the multi-disciplinary team supporting the assessment and care planning for our service users.

To act as care co-ordinator to an agreed caseload of service users with severe and enduring mental illness.

To undertake ongoing assessment, treatment and evaluation for service users known to the service

To support the delivery of therapeutic interventions either individually or within a group setting.

Support the transformation of the team to deliver local evidence based care.

The transformation will happen in staggered stages; Hampshire CAMHS, part of Sussex Partnership NHS Foundation Trust joined Southern Health in February 2024, with Isle of Wight NHS Trust mental health and community services transitioning in early May 2024. The final combination of Southern Health and Solent Trusts is anticipated to occur in July 2024, when the new organisation will be formed.

Our new Trust will continue to cater to the unique needs of different communities, making healthcare across the county more accessible, as well as offering staff more opportunities for career development, training and partnership working.
