Job overview

The post holder will play a key role in developing nursing expertise in Colorectal Cancer to improve experience of local population with Colorectal cancer care.

To work as an expert Colorectal cancer nurse within cancer Multi-Disciplinary Team liaising with both core and extended members To manage a caseload of patients/clients effectively in accordance with agreed National and South East London Cancer Alliance (SELCA) guidance, protocols and pathways of care To ensure that patients and their families are fully informed and supported by providing clinical, emotional, psychological and social assessment and support in accordance with Personalised Care Standards To assist in the development of evidence based clinical nursing practice in line with current research and guidelines To establish clear lines of communication within the specialty team to ensure cohesive multidisciplinary management of patients To be an integral part of established cancer team and specialist MDM, maintaining excellent communication to support coordinated care between sites To be an integral part of the established CNS Team and be willing to work closely with and provide support for all Colorectal CNS services if needed: (Early Polyp service, TAC 2WW Service and ERAS). To be act as a patient advocate at cancer MDM To link with healthcare teams in order to increase knowledge and facilitate the care of this group of people in the community

Main duties of the job

Fully comply with the trust Cancer CNS Standard Operational Policy  To provide specialist nursing advice, clinical assessment and support to patients, their families and other healthcare professionals following diagnosis and through treatment and where applicable onward referral for end of life care. To ensure continuity of a high standard of evidence based nursing care

This can include:

Autonomous management of cancer patients caseload Initiating investigations and reviewing results Demonstrate sound knowledge of cancer and its treatment in relation to people living with and beyond cancer Symptom management for the consequences of cancer and its treatment Develop practice to be able to deliver nurse led clinics supporting cancer patients care and needs Assessing and meeting patient’s on going care and health and wellbeing needs and develop a care plan and action referrals as a result Identify high-risk individuals and groups offering personalised care according to their individual needs Working with the CNS team to deliver end of treatment summaries and stratified follow up to improve quality of life for people living with and beyond cancer
