About us

Stanhope Mews West Surgery is a high performing GP Practice in South Kensington, serving a population of around 16,000 patients.

The practice motto is 'Helping others to help themselves' and we try to embed this in every interaction - both with patients and among the staff team.

We are a friendly workplace - informal yet professional. Every team member is encouraged to have an opinion and to voice it.

You will be working in a team of four nurses and two HCAs, in addition to the wider practice team. You will be supported to work to your highest ability.

Stanhope Mews West Surgery is part of Brompton Health PCN - a group of 12 local practices, offering further support and learning from good practice. We are also the local lead for Asylum Seekers - providing support to over 100 residents of a local hotel. Our nursing team are a key part of this support.

We are a training practice, and currently have three GP trainers and trainees. Most of our sessional GPs have trained with us, and have wished to stay. We embed a learning culture throughout.

Job responsibilities

Direct Patient Care:

Conduct patient consultations, including taking medical histories and performing physical examinations.Administer vaccinations, including childhood immunisations, travel vaccines, and flu shots.Perform diagnostic tests, such as blood pressure monitoring, blood tests, and ECGs.Provide wound care, dressings, and removal of sutures.

Chronic Disease Management:

Manage and monitor patients with chronic conditions such as diabetes, asthma, and hypertension.Develop and review individual care plans in collaboration with patients and other healthcare professionals.Educate patients on managing their conditions, including medication management and lifestyle changes.

Health Promotion and Disease Prevention:

Conduct health assessments and screenings, including cervical smears, sexual health services, and smoking cessation support.Provide health education and promotion activities to encourage healthy lifestyles.Offer family planning advice and contraceptive services.

Administrative Duties:

Maintain accurate and up-to-date patient records using the practice's electronic health records (EHR) system.Assist in the development and implementation of practice policies and procedures.Participate in audit and quality improvement initiatives to enhance patient care.

Collaboration and Communication:

Work closely with GPs, healthcare assistants, and other members of the practice team to coordinate patient care.Liaise with external agencies and services, such as hospitals and community services, to ensure continuity of care.Participate in team meetings, case conferences, and professional development sessions.

Professional Development:

Stay current with nursing practice developments, guidelines, and regulations.Participate in ongoing training and continuing professional development (CPD) activities.Supervise and mentor junior staff and student nurses as required. Person Specification

Qualifications

Essential

Registered Nurse (RN) with valid Nursing and Midwifery Council (NMC) registration.

Experience

Essential

Registered Nurse (RN) with valid Nursing and Midwifery Council (NMC) registration. Proven experience in a primary care or community nursing setting. Strong clinical skills, including phlebotomy, immunisation, and chronic disease management. Excellent communication and interpersonal skills. Ability to work independently and as part of a team. Strong organisational and time-management abilities. Proficiency in using electronic health records (EHR) and other medical software.

Desirable

Additional qualifications or certifications in areas such as chronic disease management, family planning, or health promotion. Experience with NHS systems and protocols. Knowledge of local health and social care services.
