Job summary

As a Recovery Practitioner within Delphis Therapeutic Community HMP Wymott team in Leyland, you will work closely with Offenders offering Drug and Alcohol Recovery Service to support their Substance misuse. Your role will involve being on an intensive treatment unit where men come to address their substance misuse issuesand behaviours. Responsibilities will include. 1:1 intervention, group work delivery, assessments, referrals, and overall support of their desire for change. You will work closely with the Care Coordinators and the whole DARS team. This is an exciting and rewarding role where youll be able to positively impact peoples future, improving their prospects and their quality of life.

As a Recovery Practitioner you will need to be able to work independently and within a team, within prison environment. Ensuring to deliver the best possible outcomes for those our care.

Main duties of the job

-The ability to complete substance misuse assessments on clients.

-Support and engage clients through Recovery plans.

-Facilitate 1:1 sessions supporting their overall goal.-Facilitate group sessions.

-Complete your admin duties and ensure all documentation is completed effectively and in a timely manner.

-The ability to engage with challenging individuals and build trusting relationships patience and resilience will be key

-A passion for supporting people, providing exceptional care.

-A flexible and innovative approach to service delivery

Job responsibilities

Psychosocial assessments to help create a person-centered integrated care plan.

Delivery of awareness sessions linked to substance misuse.

Joint multidisciplinary collaboration with all stakeholders such as Healthcare, offender management team, safer custody, Community CDTs and CPNs etc.

Caseload of complex and non-complex clients.

Care planning of a clients recovery journey which is person centered and puts the client at the focus of all decision making.

Effective delivery of assessments, implementation and monitoring of group work, individual psychosocial support, planning community re-integration and treatment exit.

Effectively deliver harm reduction interventions to clients.

Supporting colleagues and managers with new initiatives and service offers, and any reasonable requested Activity.

Integration within the wider calico group.
