Joint multidisciplinary collaboration with all stakeholders such as Healthcare, offender management team, safer custody, Community CDTs and CPNs etc. Caseload of complex and non-complex clients.

Care planning of a clients recovery journey which is person centered and puts the client at the focus of all decision making. Effective delivery of assessments, implementation and monitoring of group work, individual psychosocial support, planning community re-integration and treatment exit. Effectively deliver harm reduction interventions to clients. Supporting colleagues and managers with new initiatives and service offers, and any reasonable requested Activity.
