Job summary

An exciting opportunity has arisen to join the HIV Team at QEHB in the new role of Service Coordinator.

Working alongside the clinical team in an outpatient setting you will provide a point of contact for patients enabling then to access support services. The post holder will support service users to complete housing and benefit applications as well as accessing peer support and third sector agencies.

The role is ideal for an aspiring social care and health professional.

Can you work on your own initiative and deal with the unpredictable? The post holder will deal professionally with enquires from staff, patients and partner agencies.

You will have a thorough understanding of confidentiality in everyday working practice.

Main duties of the job

Provide a comprehensive co-ordination service, working in conjunction with colleagues, service users and stakeholders to provide an efficient service.

Work with patients and understand own limitations.

Provide effective communication and problem solving both face to face and via telephone

Be sympathetic and sensitive to the requirements of staff, service users when communicating by telephone or face to face, including dealing with difficult situations

Contribute to building effective teamwork in exchanging views, ideas and communicating effectively.

Ability to recognise situations that should be referred onto the Line Manager and take prompt and appropriate action.

The role will cover core clinic hours -- Monday to Friday including last Wednesday evening to 8pm
