District Community Nursing Sister/Charge Nurse

Ideally the candidate will bring with them clinical experience of working with adults in a community setting. Your working hours will be Monday to Friday (with some weekend and late shifts work on a rotation basis). We welcome applicants who would like to work part-time or on a job share. A full UK driving licence and the use of a car for work purposes is essential for this role.

Main duties of the job

We are seeking an enthusiastic and experienced nurse, with a warm personality and great people skills. You will be willing to undertake relevant training, have leadership experience and be able to demonstrate innovative practice. Show effective management skills, you will take responsibility for caseload management of housebound patients with multiple nursing need in the community.

You will be responsible for ensuring high quality evidence-based care for patients, their families, and carers in the community.

Responsible for a complex and diverse caseload, actively working to prevent admission to the acute sector and supporting patients in their preferred place of care. Responsible for maintaining detailed, accurate and confidential patient records, adhering to Trust policies. Leading an effective supportive team, working together to provide clinical advice and support to other staff and patients. Responsible for the management of a defined caseload of patients, with continuing responsibility for the assessment of care needs, the development, implementation, and evaluation of care programmers. Accountable for the standards of care and for determining the activity of your team. This will include acting as a role model for staff, sharing learning and developing practice.

Registered Nurse - Adult Active on the Mentor Register Evidence of Leadership & Management development Recognised coaching qualification or willing to undertake

Desirable

Specialist Practitioner qualification (DN Degree, diploma or Certificate in District Nursing) Non-Medical Prescriber/Nurse Prescriber Qualification

Continuous Professional Development

Evidence of continuing professional development (CPD) Evidence of a current CPD portfolio

Previous Experience

Essential

Experience in mentoring and assessing both pre and post reg. nurses in a community setting. Evidence of clinical leadership within a community nursing environment including Mentoring/assessing in practice Recruitment and selection Risk and Incident Reporting (Root Cause Analysis) and supporting teams in learning from findings. Clinical Leadership course or willingness to undertake Able to manage own workload and support others Able to conduct courageous conversations and support staff with performance related issues At least 2 years Community Nursing experience at Band 6. Experience of leadership within a team.

Understanding of the role of Community Nursing Able to work autonomously High levels of effective communication, influencing and interpersonal skills. Good organisational skills. Able to motivate self and teams to positively manage change Reinforce within the service the need to view users holistically and promote self-care and independence. Work collaboratively with their counterparts from teams in other provider organisations to develop a seamless service. Effective role model and motivates staff within the service by being highly visible, approachable and communicative at all levels; utilising effective written, oral and presentation skills. Develops strong interpersonal relationships, ensuring that staffs are given the support to develop and grow into roles. Ability to support staff to undertake clinical duties, implementing evidence-based practice. Ability to influence and problem solve. Creative and innovative approach to service development Applies policies and procedures effectively to manage risk Demonstrates cultural awareness Demonstrates flexibility to deal with the needs of unscheduled services and able to prioritise workloads and work under pressure. Understands the business needs of the organisation and shares all available information with the service ensuring people understand latest developments and their relevance Knowledge of current DH documents/government legislation and recommendations. Willingness to undertake further qualifications and develop expertise in identified areas. Participation in audit. Able to utilise IT and various media in order to support data collection

Additional Requirements

Able to demonstrate recent innovative practice. Able to work flexibly across service areas to enhance quality of service provision. Valid full UK licence with confidence to drive during and in unusual environments with use of car for work purposes. A degree of agility and the ability to manoeuvre light to moderate loads. Able to work bank holidays, weekends and unsocial hours to meet service needs. Able to work in challenging environments.
