Job summary

Exciting opportunity has arisen for a Band 7 Patient Safety Practitioner role to join the Corporate Patient Safety team.

We are looking for a highly motivational individual to join the team, who possess excellent communication and leadership skills, to support taking forward the patient safety agenda at UHDB. Particular focus will be the deteriorating patient, sepsis and VTE.

The role involves working across all the UHDB sites, with all clinical staff supporting and facilitating improvement across the patient safety workstreams.

Main duties of the job

The Patient Safety Practitioner will support the Head of Patient Safety to deliver on the patient safety elements of the Trust strategies, as well as working Trust Clinical lead for VTE, Thrombosis Committee and Anticoagulant Service. This will involve:

Developing a culture of reviewing patient safety incidents to identify learning and lead on improvements. Provide direction, expertise, advice and support to clinical staff and managers relating to patient safety. Facilitate and support staff in driving forward continuous improvement, embedding a patient safety culture across the organisation. Promote innovative and creative approaches to improving clinical quality performance. Lead on patient safety related projects, ensuring projects are established, run and monitored in line with Trust standards. To engage, facilitate and support teams to drive forward national and local initiatives to deliver and measure improvements in patient safety.
