CareNavigator

Responsible to Operations Manager

Our Care Navigators provide the first point of contact for our patientsand act as a focal point of communication between patients, doctors and othermedical staff.

Care Navigators are key members of The Quarter Jack Surgery team.They are trained to work with all the resources available to them which mayinclude signposting patients to another service, offering self help advice orbooking with an appropriate clinician in the practices multi-disciplinaryteam. This work is often referred to as care navigation.

The following are the core responsibilities of a CareNavigator. There may be on occasion arequirement to carry out other tasks; this will be dependent on factors such asworkload and staffing levels.

KNOWLEDGE, SKILLS EXPERIENCE REQUIRED

Good team working skills

Able to provide a professional and warm welcometo our patients

Ability to maintain confidentiality

Good telephone manner and inter-personal skills

Good standard of general education

Ability to work with a methodical and careful approach

Flexibility to adjust work patterns to meetadditional workload as it arises

Ability to remain calm and well-organised under pressure

IT-literate, able to use email effectively, withbasic knowledge of web-based and windows-based systems.

Able to multi-task and prioritise
