Job summary

Are you looking for a faced paced service with the ability to be involved in service development. The role of the UCRT therapist is to carry out rapid response functional assessments and interventions to support patients to remain safe and independent in their own home and support recovery. The post holders will need to have a high degree of professional autonomy and be able to make critical/clinical judgements using expert knowledge, skills and experience in order to provide a gold standard of holistic care including complex management to patients with both physical and mental health conditions. As the Band 6 you will be responsible for working autonomously during shift, feeding back to Clinical Leads within UCRT. You will be expected to contribute to the service as a whole with MDTs, service development, supporting the trust and attend service meetings.

The post requires you to be able to carry equipment and travel though out the wholearea commissioned.

Main duties of the job

Clinical responsibilities

Undertake comprehensive, holistic assessments of individuals, including those with a complex presentation, using investigative, analytical & clinical reasoning skills, and formulate a treatment plan.

Undertake functional assessments for patients, by using investigative and analytical skills for those with acute/complex/rehabilitation or palliative care needs

Liaise with GPs, Social Services, inpatient teams, other allied health care professionals and the voluntary sector, to ensure identified needs are met and care co-ordinated appropriately.

Communicate effectively with individuals and their carers to maximise potential, and to aid motivation and understanding throughout the treatment process.

Empower/enable patients and carers to maximise potential for self-care.

Required to use tact and persuasive skills in order to gain the individual's cooperation in their treatment plan.

Be responsible for ensuring workload for self and others is planned and prioritised according to service targets and needs.

Ensure patient held records are completed for each visit, and that SystmOne inputting is completed on a daily basis.

Where safe care is not able to be sustained to liaise with the wider community services and care coordination centre to plan and implement appropriate actions

Job description

Job responsibilities

The therapist must be able to adapt their communication, treatment, information and advice to meet the needs of individual patients. Patients seen will range from the vulnerable to those who are extremely well informed about their condition. The therapist must be sensitive to patients own cultures, beliefs and emotions, especially when asking them to undress for treatment n during assessments or treatments, which are likely to include the use of hands-on physiotherapy skills. Consent must always be obtained prior to assessment and treatment. The therapist must take a holistic view of the patients and their wellbeing considering all the factors and not purely focus on a specific medical condition, sign or symptom. To be professionally and legally accountable for all aspects of own wok including the management of patients in your care. To take a lead for specific projects within your area of work to enhance patient care and service delivery. To train, supervise and performance manage junior staff, technical instructors, assistants and students (some on a daily basis) with assistance from more senior members of the physiotherapy team when needed. This will include the use of formal appraisal documentation. To effectively manage and prioritise your own workload within the time available including the prioritisation of patients to be treated along with other commitments in the day. Actively participate in Continuous Professional Development and maintain a portfolio recording learning outcomes. Undertake the measurement and evaluation of your work and current practices and participate in research and development through the use of Evidence Based Practice projects, audit and outcome measures, either individually or with more senior physiotherapists. To make recommendations for change as a result of this process. Demonstrate a sound understanding of Clinical Governance and Risk Management and apply this understanding to work situations.
