Job summary

NHS Somerset ICB currently has an exciting opportunity foran experienced administrator to join the ICB Safeguarding Team as the SomersetChild Death Review Coordinator. This a 12 month fixed term post to providecover for the substantive postholder who is on secondment.

The post holder will be working closely with the DesignatedProfessionals for Child Death and Safeguarding Children, coordinating the childdeath review process on behalf of the Somerset system, from point ofnotification of a childs death to the Child Death Review being scrutinised bymulti-agency professionals at the Child Death Overview Panel (CDOP).

This is a varied and interesting role which can bechallenging at times, but is ideal for someone who is able to use their owninitiative, work independently, but also enjoys having the support of a larger safeguardingteam.

Main duties of the job

You will: Be responsible forgathering information from a range of professionals including health, socialcare, the police and the coroner, and have the ability to apply stronganalytical skills to identifying themes and trends. Ensure businesspriorities are met by maintaining records, creating files, minute taking, datainputting and collection of information via eCDOP. Data which may bedistressing and is highly confidential. Assist in thepreparation and production of reports and papers for the Child Death Reviewcase meetings, CDOP and the CDOP annual report. Be a key member of thePan Dorset and Somerset Child Death Overview Panel (CDOP), supporting SomersetICB, Somerset Council and Child Death Review partners in Dorset in meeting ourstatutory duties as detailed in Child Death ReviewStatutory and Operational Guidance. Support CDOP members tolook at the circumstances around every child death and determine what can belearned, to prevent further deaths and better support families and communitiesaffected by the death of a child. Be required to haveregular contact with internal and external stakeholders including child deathreview partners; often needing to engage with them over sensitive, complex,contentious, and confidential issues

Purpose of the role

WorkingTogether to Safeguard Children 2023 defines child death review partners aslocal authorities and any ICB for the local area as set out in the ChildrenAct 2004 (the Act), as amended by the Children and Social Work Act 2017.

When a childdies, in any circumstances, it is important for parents and families tounderstand what has happened and whether there are any lessons to be learned.The responsibility for ensuring child death reviews are carried out is held bychild death review partners, as defined within Working Together 2023 and setout in current legislation. Child death review partners must make arrangementsto review all deaths of children normally resident in the local area and, ifthey consider it appropriate, for any non-resident child who has died in theirarea.

TheChild Death Review Coordinator will ensure these arrangements functioneffectively.

Key Responsibilities of the Role

Be the multiagency initial point of contact for Somerset child death notifications, via eCDOP (the electronic system used) or via telephone Be the multiagency point of contact for Somerset child death reviews, via email or via telephone Assess and distribute sensitive child death information followingnotification to multi-agency partners Escalate concerns where key information is outstanding within thenotification process Implement statutory specialist process for information gathering Investigate and identify multiagency practitioners who could potentially be involved with a child. Make contact with wide range of practitioners via email and telephone to explain the child death review process. Send statutory reporting form requests via eCDOP Collate reporting form information when returned, track and follow up on outstanding forms Analyse each reporting form for any gaps in information, anyadditional professionals identified.Ensure all multi-agency involvement is captured and requests forinformation have been instigated. Flag any apparent risks or concerns upon receipt ofinformation. Escalate any issues toChair / Designated Doctors Child Death Liaise with coroner and coroners officers about post mortemreports and inquests Co-ordinate and track rapid response information gathering. Arrange, administer and attend the multi-agency meetings the DesignatedDoctors Child Death hold for each child, synchronising dates and venues includeestablishing who needs to be invited. Liaise with out of area Acute Hospitals about children who have diedin their care, send statutory documentation to them in order to plan theirmultiagency meetings. Facilitate the Somerset Designated Doctors Child Death attendance atmulti-agency meetings in person or by teleconference. Propose which cases are ready to be taken to the CDOP. Prepare agenda for CDOP panel on behalf of the Chair. Prepare CDOP documentation, anonymising all case records andamalgamating all information collected. Organise, attend and minute CDOP meetings with Dorset CDOPCoordinator Meet with Chair after panel meetings to monitor progress of actiontracker Maintain a log of actions arising from panel meetings. Trackactions and follow up outstanding actions with multiagency partners. Provide support to Chair including sending letters following panelmeetings. Prepare agenda and associated papers for the Somerset Child DeathReview (CDR) team meeting on behalf of the Designated Nurse SafeguardingChildren (Chair). Organise, attend and minute the Somerset CDR team meetings. Meet with Designated Nurse Safeguarding Children after the SomersetCDR team meeting to monitor progress of action tracker Maintain a log of actions arising from the Somerset CDR teammeetings. Track actions and follow up outstanding actions with the team. Provide support to Chair. Liaise with regional and national CDOP coordinators about sharedcases, themed reviews and distribution of national learning within Somerset National point of contact for CDOP panel to cascade learning fromother panels or to answer questions Provide ad hoc safeguarding children admin cover to the Designated/DeputyDesignated Nurse, Doctor and Named GP for Safeguarding Children in the absenceof the part time Safeguarding Children Administrator.
