Larksfield and Arlesey Medical Partnership has a fantastic opportunity for respiratory/diabetes nurse to join our team.
Main duties of the job
Undertaking health assessments, administering medication and treatments, and performing diagnostic tests. Providing and evaluating a range of health interventions to ensure that patients health and social care needs are met. General health and well-being promotion as well as creating individual, person-centred plans for patients with, or at risk of, a long term condition. Managing and treating long-term conditions, in line with national and local policies and the practice needs. Reviewing medication for therapeutic effectiveness, in accordance with evidence-based practice as well as national and practice protocols; working with patients to support their adherence to prescribed treatments. Prioritising health problems and intervening appropriately, to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care. Supporting / managing and reviewing the health needs of patients with diabetes and/or respiratory illness such as asthma and COPD. Perform spirometry testing.
About us
We have a patient population of approx. 19,500 across our main site (Larksfield surgery) and branch site (Arlesey Medical Centre). We have anexcellent clinical and administration team with a highly skilled MDT team including GPs, nurse practitioners, a clinical pharmacist, paramedic practitioners, our practice nursing team and HCAs. Person Specification
Qualifications
Essential
A relevant nursing degree. Current NMC registration with an active PIN. Fundamentals of Practice Nursing course (General Practice Nurse Foundation Programme) or equivalent experience or qualifications; Competence to undertake asthma, COPD and diabetic clinics and reviews, holding relevant certificates.
Experience
Essential
Evidence of clinical skills and experience in treatment room consultation, management of long-term conditions, and recording spirometry. Evaluating and filing HBA1c results and advising patients the best course of action / treatments. Compassionate manner with an empathetic approach and demonstrable kindness in all aspects of your work. The successful candidate will value the dignity of our patients, their relatives and carers, as well as their colleagues at the practice. Knowledge, and experience of its application, in supporting long-term conditions. Insight and understanding of current issues in nursing and the wider NHS.-
