Job summary

The Specialist Autism Team was firstestablished in May 2021. Since then the team has continued to grow and develop. The service is commissioned to offer an Inreach, Outreach and Discharge Facilitation.

We want to hear from you if you are highly committed to working with autistic adults. We are offering an opportunity for a registered nurse (Mental Health or Learning Disability), AHP or social worker to join our multi-disciplinary team as part of our discharge facilitation pathway.

Within this pathway, our primary role is to support autistic individuals back into the community, planning discharge collaboratively from the point of admission in a timely, well planned and sustainable approach. You will also support the individual and their support networks for 6 weeks post discharge to ensure their discharge is safe.

You will work closely with the Multi-disciplinary Team of the Specialist Autism Team which consists of Speech and Language Therapists and Assistants, Occupational Therapists, Learning Disabilities Nurses, Mental Health Nurses, Psychology and Assistant Psychology and wider Multi-disciplinary team such as Local Authority, Key Workers and the Integrated Care Board (ICB).

The team prides it's self on the diverse knowledge of each professional. We are very supportive of and encourage continuous learning and development.

Main duties of the job

The Service strives to provide forward thinking high quality services, working in partnership with other agencies. In addition to working with autistic people and their families we have a strong commitment to public health, promotion of safeguarding and effective risk management through service development and training initiatives working within an enthusiastic and experienced team.

A Registered Professional, with up to date knowledge, of health policy and legislation applicable to autistic people. Adaptability, good communication skills and self-motivation are essential. An experienced clinician with proven skills in working with people who have complex health needs and challenging behaviour and autism, demonstrating practice is evidence based and innovative.

The post holder will be responsible for a team caseload and supervision of other practitioners (registered and unregistered), to provide strong clinical leadership and supervision within the Specialist Autism Team.

The post holder will work flexibly between the hours of 8am and 8pm by working a rotational shift pattern including weekend working to meet the needs of the team and the wider network.
