Discharge Facilitator for Autistic Adults

About the Role

We are seeking a dedicated and experienced Discharge Facilitator Practitioner to join our Specialist Autism Team. This multi-disciplinary team is committed to providing high-quality, forward-thinking services to autistic adults within Lancashire & South Cumbria.

As a Discharge Facilitator, you will play a crucial role in supporting individuals back into the community following their admission. You will work collaboratively with the individual, their support networks, and the wider multi-disciplinary team to ensure a safe and sustainable discharge plan.

Key Responsibilities

  • Collaborate with autistic individuals, families, and healthcare professionals to develop comprehensive discharge plans.
  • Provide ongoing support and guidance to individuals for six weeks post-discharge, ensuring a smooth transition back into the community.
  • Work closely with a diverse team of professionals, including Speech and Language Therapists, Occupational Therapists, Learning Disabilities Nurses, Mental Health Nurses, Psychologists, and Local Authority representatives.
  • Demonstrate strong clinical leadership and supervision skills, mentoring and supporting other practitioners within the Specialist Autism Team.
  • Maintain up-to-date knowledge of contemporary autism policy, legal frameworks, and best practices in discharge planning.

About You

We are looking for a Registered Professional (NMC or HCPC registered) with proven experience working with individuals who have complex health needs and challenging behaviors. You will possess excellent communication, interpersonal, and problem-solving skills, along with a commitment to providing person-centered care.
