Main duties of the job

This new role will see thesuccessful Occupational Therapist candidate delivering skilled assessments andtherapeutic interventions to patients in their home, promoting a palliativerehabilitation and enablement approach, embracing positive risk taking andlistening to what matters to the individual and their family. As part of the Rehabilitation and EnablementTeam, working closely with the Mountbatten Community Nursing Team, thepostholder will manage their own caseload, whilst being well supported in morecomplex cases with opportunities for learning and joint working.

The Occupational Therapist will takeinto account cultural and spiritual needs, and share their learning with thewider team. As this is a funded post, they will also support with essentialdata collection, audit and evaluation, to prove the impact of this novel role,with the aim of attracting ongoingfunding and an extension of a longer-term or permanent contract.

This vacancy isfor 30 hours per week to be worked flexibly in accordance with the needs of theservice.

Job responsibilities

Key tasks:

Toperform advanced Occupational Therapy assessment of patients with complex anddiverse palliative care needs in their own home. To provide, develop anddeliver individualised interventions, and to undertake these tasks as anautonomous practitioner.

Maintain a focus on optimising patients functional andself-management abilities and enable them with what matters most to them,through tailored and appropriate interventions, including functional practice,provision of aids/equipment, education and non-pharmacological symptommanagement.

Provide moving and handling advice and support tostaff, patients, families and carers to enable the individual, enhance qualityof care and reduce carer burden within the home.

To facilitate and promote a palliativerehabilitation and enablement philosophy and approach throughout theorganisation and to otherhealth and social care providers.

Alongside colleagues, coordinate and develop the dayto day operation of the Hospices Community Team, considering skill mix andpriority of need.

Act as an Occupational Therapy palliative careresource; providing consultative advice, education, and support to members ofthe Hospice Multidisciplinary Team (MDT) and Health and Social Care teams.

Promote and support the development of this new andinnovative role with the support of the Head of Rehabilitation and Enablement.

Qualifications

Essential

Degree/Diploma in Occupational Therapy Current registration with Health Care Professional Council

Experience

Essential

Understanding of a palliative rehabilitative and enablement approach Understanding of Clinical Governance and the implications for therapy services including experience of quality issues and audit Experience of supervision of OT support staff or students

Skills

Essential

Articulate and able to clearly record professional activity in patient records, reports, etc. Current driving full licence and access to a vehicle for business use

Abilities

Essential

Understanding of a palliative rehabilitative/enablement and application to practice Demonstrate an enthusiastic, approachable manner using an empowering/enabling approach being sensitive to the needs of others
